Embodiments herein relate to ear-wearable stress and anxiety monitoring systems, devices and methods. Embodiments herein further relate to ear-wearable systems and devices that can detect and take actions to alleviate device wearer's stress and anxiety. In an embodiment an ear-wearable stress and/or anxiety monitoring system is included having a control circuit, a microphone, and a sensor package that can include a motion sensor. The ear-wearable system is configured to evaluate data from at least one of the microphone and the sensor package and classify a stress level of a device wearer using a machine learning classification model and periodically update the machine learning classification model based on indicators of stress experienced by the device wearer.
Implantable medical device and method for managing advertising and scanning schedules
Methods and devices for managing establishment of a communications link between an external instrument (EI) and an implantable medical device (IMD) are provided. The methods and devices comprise storing, in memory in at least one of the IMD or the EI an advertising schedule defining a pattern for advertisement notices. The advertisement notices are distributed un-evenly and separated by unequal advertisement intervals. The method transmits, from a transmitter in at least one of the IMD or the EI the advertisement notices. The advertisement notices are distributed as defined by the advertising schedule. The method establishes a communication session between the IMD and the EI.

Cardiac modulation facilitation methods and systems
A method of facilitating therapeutic neuromodulation of a heart of a patient includes positioning an electrode in a pulmonary artery, positioning a sensor in vasculature, delivering via a stimulation system first and second electrical signals of a series of electrical signals to the electrode. The second electrical signal differs from the first electrical signal by a magnitude of a first parameter of a plurality of parameters. The method includes determining, via the sensor, sensor data indicative of one or more heart activity properties in response to the delivery of the series of electrical signals, and delivering a therapeutic neuromodulation signal to the pulmonary artery using selected electrical parameters. The selected electrical parameters include a selected magnitude of the first parameter. The selected magnitude of the first parameter is based at least partially on the sensor data. The therapeutic neuromodulation signal increases heart contractility.
Method and apparatus for stimulation
A medical device system and method are disclosed for treating obstructive sleep apnea. The system includes a pulse generator and a medical electrical lead including multiple electrodes carried by a distal portion of an elongated lead body. The method includes advancing the distal portion within protrusor muscle tissue below the oral cavity and delivering electrical stimulation pulses via the electrodes to sustain a protruded state throughout a delivery time period to sustain a protruded state of a patient's tongue throughout the therapy delivery time period. The therapy delivery time period may span multiple respiration cycles.

Prediction of worsening of heart failure using blended reference
Systems and methods for detecting cardiac conditions such as events indicative of worsening of heart failure (HF) are described. A system can receive a physiological signal from a patient, transform one or more first portions of the physiological signal into respective one or more baseline statistical values, transform one or more second portions of the physiological signal into one or more historical extreme values, and generate one or more reference values of a physiologic parameter using the baseline statistical values and the historical extreme values. The system can transform one or more third signal portions of the physiological signal into respective one or more short-term values, and produce a cardiac condition indicator using a combination of relative differences between the short-term values and the corresponding reference values. The system can output the cardiac condition indicator, or deliver therapy according to the cardiac condition indicator.
