The applicator is operable to be held for enabling the projected out insert to be inserted into second end for detachably coupling the first and second ends to form a closed loop around the fistula track.

A method and apparatus for rectal fistula laser treatment wherein an insertion tube of a coupler is inserted into a fistula from proximal to distal end. The insertion tube is then purged using sterile water and/or air. A flexible optical waveguide is then inserted through the coupler including the insertion tube until the end of the optical waveguide protrudes 1-3 mm from the distal end of the insertion tube. The optical waveguide is secured to the coupler by a lock nut of the coupler. The insertion tube locked to the optical waveguide is withdrawn through the fistula while CO2 laser radiation is passed down the optical waveguide so that laser radiation is delivered to the inner wall of the fistula at a specific rate as the laser radiation treats the fistula.

Current invention relates to a combined system developed for the purpose of use in the treatment of anorectal fistulas. Fistulectomy Set is a system consisting of fistulectomy units which functions fully controlled and transmitting the pivoted movement which comes from the movement providing and transmitting part to the cutting edge by moving it on a central guide with the help of a motion carrier flexible shaft consisting of hexahedron spheroid parts. It consists of a cannulation apparatus which provides a simultaneous rotation, vibration and reciprocating movement and which detects the fistula trace fully and accurately. With this cutting edge, a channel surrounded by living tissue is obtained by cutting around the fistula trace cylindrically with suitable thickness, and thus cutting the secretory mucosa, dead tissues and fibrotic structures. With the developed system, it is believed that important progress will be made in the treatment of anorectal fistula; an important social and medical problem. It will prevent the loss of labor force, shorten hospital stays, reduce costs and minimize complications of treatment. Most importantly, it will prevent anal incontinence and hence a very important problem will be solved.

The apparatus comprises an adapter for a medical instrument, the adapt having a body having a shim portion and an instrument holding portion adjacent the shim portion, wherein the shim portion provides an enlarged peripheral surface adjacent the medical instrument such that when the medical instrument is held in the instrument holding portion and the medical instrument and body are inserted through an opening in a patient tissue, at least a portion of the enlarged peripheral surface is in contact with at least a portion of the periphery of the opening in the patient tissue.

Fistula grafts and related methods and systems useful for treating gastrointestinal fistulae

Described are medical graft products, systems, and methods useful for treating fistulae, particularly enterocutaneous fistulae. Certain products of the invention are configured to have portions residing in and around a primary fistula opening in a wall of the alimentary canal. One such product includes a biocompatible graft body which is configured to block at least the primary opening. The graft body includes a capping member connected to an elongate plug member. The capping member is configured to contact portions of the alimentary canal wall adjacent to the primary opening, and the elongate plug member is configured to extend into at least a portion of the fistula. A graft product of this sort may be particularly adapted to allow a portion of the capping member to be positioned alongside an exterior, lateral surface of the plug member, e.g., when placed in a delivery device lumen. Such a capping member may be hingedly or non-hingedly coupled to the elongate plug member.
