A method and set of instruments are disclosed particularly useful for carpal tunnel surgeries that allow a precision cut in the transverse carpal ligament (TCL) without direct vision or exposure of the ligament, except for its most proximal edge, but with guidance and safety of the cutting knife, eliminating or, at least very much decreasing, the probability of cutting lines in the wrong direction and inadvertent (iatrogenic) lesions to the surrounding structures. In one preferred embodiment the instrument comprises: a uniquely shaped cannulated guide rod, through which passes a flexible metal guide needle (33) which serves as a guideline for a uniquely shaped cutting knife or fasciotome (25) having a cannulated finger-like prong in the inferior edge of the blade portion of the knife.

A surgical instrument including a handle portion, a body portion, an articulating tool assembly and a locking assembly is disclosed. The handle portion includes a movable handle. The body portion extends distally from the handle portion and defines a first longitudinal axis. The articulating tool assembly defines a second longitudinal axis and is movable from a first position where the second longitudinal axis is substantially aligned with the first longitudinal axis to at least a second position where the second longitudinal axis is at an angle to the first longitudinal axis. The locking assembly includes a member that is advanceable distally with respect to the body portion. The member engages the articulating tool assembly upon manipulation of the movable handle to move an anvil and a cartridge assembly in approximation with one another to help maintain the articulating tool assembly in its first position.

KNIFE AND RETRACTOR SYSTEM

A gastrocnemius knife and retractor system and kit includes a surgical knife and a soft tissue retractor. The soft tissue retractor is configurable to move from a collapsed mode to an expanded mode and back again, and can be inserted through a small incision when in the collapsed mode. Once inside the incision, the soft tissue retractor is expanded to permit visualization and accessibility of the surgery site. The surgical knife is then introduced and guided along a path in the frame of the soft tissue retractor, thereby incising the desired tissue. Once the tissue is incised, the surgical knife is withdrawn, followed by the soft tissue retractor after it is collapsed.

Urethrotome dilators for dilating a urethral stricture, and methods for forming and using the same are disclosed. In some embodiments, the urethrotome dilator includes a first distal end with a dilating head, a second proximal end with a handle, and a stem for connecting the first end to the second end. The dilating head may have a first region, a second region, and a channel for receiving a guidewire. The first region of the dilating head may have a substantially conical surface, with a vertex at its distal end and a base at its proximal end. The second region may have a substantially flat surface and a blade for stretching tissue. The blade may be integrally formed within the second region and may extend from the flat surface. The top edge of the blade may be tapered and blunt, while the two opposing sides of the blade may be formed with sharp corners for cutting tissue.

A probe unit includes, a probe configured to treat a bone by ultrasonic vibration, a hollow sheath which surrounds the probe and which has a first portion at a small distance from a central axis, and a second portion at a greater distance from the central axis than the first portion, and a knob configured to rotate the sheath relative to the probe between a first position for insertion between the bone and a living tissue facing the bone so that the first portion is located between the bone and the living tissue and a second position for insertion between the bone and the living tissue so that the second portion is located between the bone and the living tissue.

ENDOSCOPIC TREATMENT TOOL

An endoscopic treatment tool includes: a tubular insertion part; a stopper having a through hole and provided at a distal end part of the insertion part; a manipulation wire inserted through the insertion part; a treatment part provided at a distal end part of the manipulation wire and inserted through the through hole; and a restriction member provided closer to a proximal end side than the stopper and connecting the treatment part and the manipulation wire. When the restriction member is pressed against the stopper, the treatment part is locked not to be rotated about an axis of the insertion part with respect to the insertion part, and an opening causing a fluid to flow from an inside of the insertion part to the through hole is formed on an outer peripheral surface of the restriction member.

A dissecting device for dissecting tissue surrounding a blood vessel in a living body. The dissecting device includes a grasping section that includes an insertion lumen. The dissecting device includes a dissecting member at the distal portion of the grasping section. The dissecting member is insertable into the living body along the blood vessel to dissect the tissue surrounding the blood vessel in the living body. The dissecting member includes a treating section and a protruding section. The treating section performs a predetermined treatment of a branch vessel branching from the blood vessel. The protruding section protrudes from the treating section in the thickness direction of the treating section. The protruding section includes a guide section configured to guide the branch vessel toward the treating section.
