A surgical device includes a handle assembly, an elongated portion, an end effector, a visualization device, and a constant horizon mechanism. The elongated portion extends distally from the handle assembly and defines a longitudinal axis. The end effector is rotatable about the longitudinal axis relative to the handle assembly. The visualization device defines a visualization axis. A first portion of the visualization device extends through the elongated portion, and a second portion of the visualization device is disposed at least partially within the handle assembly. The visualization device is rotatable about the longitudinal axis relative to the handle assembly. The constant horizon mechanism is disposed in operative engagement with the visualization device and is configured to prevent the visualization device from rotating about the visualization axis when the visualization device rotates about the longitudinal axis.

A system for creating an arteriovenous (AV) fistula comprises a vessel access sheath having a hollow interior and an exit port, a side access needle catheter configured to fit within the hollow interior of the sheath, a needle configured to be inserted into a blood vessel through the side access needle catheter, a toggle delivery catheter configured to fit within the hollow interior of the sheath, and a toggle apparatus configured to be delivered into a vessel through the toggle delivery catheter. The toggle apparatus comprises a shaft and a toggle member pivotably attached to a distal end of the shaft. A source of RF energy or resistive heat energy may be provided for application to the toggle member and/or to a heater insert in the toggle delivery catheter, for the purpose of creating the fistula.

Selection of graphical elements and/or between graphical elements can cause activation of the set of transducers associated with the selected elements. Transducer activation characteristics, such as initiation time, activation duration, activation sequence, and energy delivery characteristics, can vary based on numerous factors. Visual characteristics of graphical elements and between graphical elements can change based on an activation-status of the corresponding transducers. Activation requests for a set of transducers can be denied if it is determined that a transducer in the set of transducers is unacceptable for activation.

Provided herein are expandable devices, rail systems, and motorized devices. In one embodiment, an expandable device comprises an expandable sac having a tool housed therein. The expandable device is optionally configured for operation while inside a body cavity. The expandable device optionally comprises at least one rail in the sac, and at least one railed device coupled to the rail for movement there on. Movement of the railed device on the rail is provided by, for example, a motor such as an electromagnetic motor or an inch-worm type motor. Expandable devices can be used, for example, to perform minimally invasive medical procedures requiring access to a body cavity. Expandable devices can also be used, for example, to provide safe and stable transport of instruments to the body cavity.

Surgical forceps

A forceps includes an end effector assembly having first and second jaw members movable between a spaced-apart position and an approximated position for grasping tissue therebetween. A knife assembly having a cutting blade disposed at a distal end thereof is also provided. The knife assembly is translatable relative to the end effector assembly between a retracted position and an extended position, wherein the cutting blade extends between the jaw members to cut tissue grasped therebetween. The knife assembly includes a proximal component and a first distal component that includes the cutting blade. The proximal and first distal components are removably coupled to one another to facilitate replacement of the first distal component. Methods of preparing such forceps for reuse are also provided.

When the jaw members are in an approximated position, power is delivered to the thermal cutting element to maintain the temperature of the thermal cutting element at an operating temperature for cutting sealed tissue grasped between the jaw members. When the jaw members are in the spaced apart position, power is delivered to the thermal cutting element to maintain the temperature of the thermal cutting element at a standby setpoint temperature. In response to a determination of contact between tissue and the thermal cutting element when the jaw members are in the spaced apart position, power is delivered to the thermal cutting element to increase the temperature of the thermal cutting element from the standby setpoint temperature to a cutting setpoint temperature for dissecting tissue.
