Once a desired loop size is formed between tissue faces by pulling the suture through one of the intermittent apertures in the suture thread, the suture thread is clipped and a further loop is begun using the remainder of the suture thread affixed to the suture needle.

A lead puncture needle includes a core rod, a puncture needle, a lead mechanism and a handle. A front end of the core rod includes a blunt head and a lead slot. During puncture, the puncture needle is exposed. After the puncture, the blunt head protrudes out of the puncture needle to avoid injury to normal tissue. When a button is pressed, the core rod moves downward, and the lead slot protrudes from the puncture needle. When the button is pressed again, the core rod moves upward, and the lead slot is retracted into the puncture needle to form traction on a surgical suture. The design of the blunt head ensures that it is retracted during puncture to expose the puncture needle to facilitate puncture and the blunt head is controlled to be exposed outside the puncture needle in a separation process, thereby avoiding accidental injury to the surrounding tissue.

A minimally invasive surgical method of approximating tissue includes coupling an adjustable loop of suture around an inserter. The loop can be coupled to the inserter by applying tension to a tail of the suture to collapse the loop around the inserter, then the inserter and loop are passed through a first location in tissue. The loop is decoupled from the inserter, the inserter is retracted from the first location, and reinserted into a second location in the tissue, after which the loop is recoupled to the inserter while the loop is distal of the tissue. The decoupling, retraction, and reinsertion can occur without withdrawing the inserter from the patient's body. The inserter and loop are then withdrawn together from the tissue through this second location and outside of the patient's body. The tails and loop can form a luggage knot to be reduced around the tissue.

Devices, systems, and methods are disclosed for relocating a meniscus horn in a knee. The invention may be deployed during an arthroscopic procedure to apply external forces to a meniscus horn to pull the meniscus into a load bearing area of the knee, which may reduce contact stresses on articular cartilage in the knee and thereby ameliorate pain associated with an osteoarthritic joint. In one embodiment, the system includes a guide needle, suture thread, plug, elongate member, and bone anchor.
