A tissue-fastening tool indwelling system includes: a sheath; a needle tube disposed in the sheath; a tissue-fastening tool including a wire having a coil region disposable in the needle tube in a stretched state and restorable to a coil shape with the same winding diameter outside the needle tube; a stylet connected to the wire; and a manipulation part for manipulating the stylet to advance the wire and rotate the sheath in a first direction. In a state where a distal end of the needle tube is proximal of the distal end of the sheath, the manipulation part is designed to operate such that the wire is delivered from the sheath by a length of one winding of the coil region per rotation of the sheath by less than one turn, and the protruding coil region biased to have a diameter larger than the winding diameter.

The present disclosure provides modular magnetic anastomosis devices that can be implemented in digestive surgery or in any circumstance of anastomosis between adjacent organs or two hollow viscera. The device is minimally invasive and easily and quickly delivered using laparoscopic or endoscopic procedures.

Devices and methods for assisting magnetic compression anastomosis

A positioning wand for assisting in positioning at least one of a first magnetic implant and a second magnetic implant configured for forming an anastomosis between two adjacent walls of a digestive tract of a patient is provided. The positioning wand can include an elongated member sized and configured to be inserted into an abdominal cavity of the patient, and a distal tip provided at a distal end of the elongated member. The distal tip can include a guide magnet configured to magnetically couple with the at least one of the first and second magnetic implants through a wall of the digestive tract to position the at least one of the first and second magnetic implants to a desired site of the anastomosis. The distal tip can be configured to be moveable in response to a contact pressure upon contact with the wall of the digestive tract.

METHOD FOR SHEARING TISSUE

A method is described for shearing a tissue wall between a first cavity and an adjacent second cavity in a human or animal body. The tissue wall has a periphery forming an edge of an opening between the first cavity and the second cavity. A first member is placed into the first cavity, such that the first member is adjacent the wall. A second member is placed in the second cavity, proximate the first member. The first and second members have materials which magnetically attract one another. The first member and the second member are placed such that there is magnetic attraction between the first member and the second member through the wall and thereby compress an overlapping portion of the wall to create pressure necrosis. The first member and the second member are connected by a thread, wherein the thread extends over the edge of the opening.

The device which may be attached inside or outside the pancreas/abdomen during surgery addresses leaks of pancreatic juice or pancreatic effluent (PE) after resection by redirecting and inactivating PE. Specifically the PE enzymes (proteases) are inactivated. Redirection is through an internal biodegradable self powered self eliminating drain with increasing gradient and then targeting or inactivating the effluent with active pharmaceutical ingredients (API) such as-protease inhibitors which are contained in graduated micro-encapsulated particles (GMEPs) coated with pH and time release shells which the device locates at optimum locations in the pancreas/abdomen. Another iteration is redirecting PE by containing the device within a mesh filled with GMEP comprising protease inhibitors (antiprotease) to inactivate PE before flowing out via openings in the mesh into the abdominal cavity as an inactivated innocuous fluid.

MAGNETIC ANASTOMOSIS DEVICES AND METHODS OF DELIVERY

The invention concerns delivering paired magnetic anastomosis devices to either side of tissues to be joined. The magnetic anastomosis devices are coupled to a guide element that facilitates delivery and manipulation of the devices when using minimally-invasive techniques such as endoscopy and laparoscopy. Elongated manipulators and guide tubes are also disclosed that improve a user's dexterity with the devices during placement.
