Streamlined treatment of clot removal, angioplasty and prevention of restenosis using a single integrated intravascular device
11266426 · 2022-03-08 · ·

A single integrated intravascular device including a stentriever and semi-compliant balloon housed therein. After traversing a clot, the device is deployed to a self-expanded state engaging the clot therein, whereupon the device along with the embedded clot is removed. Detecting through imaging a stenosis at an original position of the captured clot, the device is reintroduced to that location and the stentriever is deployed to a self-expanded state. Inflating the semi-compliant balloon enlarges the stentriever to a hyper-expanded state greater than the self-expanded state thereby dilating the vessel while simultaneously completely detaching/releasing the stentriever from a remaining portion of the device. Then the semi-compliant balloon is collapsed and withdrawn along with the remaining portion of the device, while the detachable/releasable portion of the stentriever in the self-expanded state remains in the vessel.

ELECTRICALLY ENHANCED RETRIEVAL OF MATERIAL FROM VESSEL LUMENS

Retrieval of material from vessel lumens can be improved by electrically enhancing attachment of the material to the thrombectomy system. The system can include a catheter having a distal portion configured to be positioned adjacent to a thrombus in a blood vessel, an electrode disposed at the distal portion of the catheter, and an interventional element configured to be delivered through a lumen of the catheter. The electrode and the interventional element are each configured to be electrically coupled to an extracorporeal power supply.

APPARATUS AND METHOD FOR TREATMENT OF POST THROMBOTIC SYNDROME

A medical apparatus has a thrombectomy device disposed in a sheath lumen of a sheath that is movable between an initial extended position and a retracted position. The thrombectomy device includes a first elongate cannula having a distal tip, a guidewire lumen, and a perforated region, which is proximal to the distal tip and is in fluid communication with the guidewire lumen. A second elongate cannula, which is configured to rotate, has a second lumen and a mechanical thrombectomy whisk, which is proximal to the distal tip. A third elongate cannula has a third exterior surface, a third lumen, an inflation passageway, an inflatable balloon connected to the third exterior surface, and an expandable embolic filter connected to the third exterior surface at a location distal to the inflatable balloon. The first elongate cannula is disposed in the second lumen. The second elongate cannula is disposed in the third lumen.

METHODS AND APPARATUS FOR TREATING EMBOLISM

A method and apparatus for treating a clot in the blood vessel of a patient, and particularly the treatment of a pulmonary embolism is disclosed. The treatment includes restoring flow through the clot followed by clot removal, either partially or substantially completely. The clot treatment device is expandable into the blood vessel and may contain radial extensions that assist in restoring flow as well as in removing clot material.

MEDICAL INSTRUMENT HAVING A BALLOON AND AN EXTRACTION MEMBER

A medical instrument may include a tube, a sheath provided around and coaxial with the tube, and an inflatable balloon at a distal end of the sheath. The balloon may receive a fluid from a lumen of the sheath, and may have a central opening along a longitudinal axis thereof, and into which the tube extends, a proximal portion, a middle portion adjacent to and distal of the proximal portion, and a distal portion adjacent to and proximal of the middle portion. When the balloon is inflated with the fluid, a maximum diameter of the middle portion may be less than a maximum diameter of the proximal portion, and less than a maximum diameter of the distal portion. The medical instrument may also include an extraction member provided at a distal end of the tube, distal to the balloon, and movable axially relative to the balloon.
