Catheter with retractable sheath and methods thereof
A method for using an endoluminal device to modify an intravascular lesion includes providing an ultrasound-producing mechanism that converts an electric current into vibrational energy at an ultrasonic frequency; providing a sheath including a sheath lumen, wherein the sheath is configured to retract from a first, fully extended position of the sheath and extend from a second, fully retracted position of the sheath; providing a core wire disposed within the sheath lumen of the sheath, the core wire being coupled to the ultrasound-producing mechanism via a sonic connector, the core wire being excited by the vibrational energy at the ultrasonic frequency when the ultrasound-producing mechanism is activated; and retracting the sheath relative to the core wire to expose a working length of a distal portion of the core wire for ultrasound-based modification of one or more intravascular lesions.
Blade grounding mechanisms and alternative pin designs
A surgical instrument includes a body, a shaft assembly, and an end effector. The end effector includes an ultrasonic blade and a clamp arm movably coupled with the shaft assembly. The shaft assembly extends between the body and the end effector and includes an acoustic waveguide, a rotational driver, and a driver wrench. The rotational driver is configured to be received within the rotational drive channel and rotate the shaft assembly relative to the body. The acoustic waveguide includes a notch and the driver wrench includes a key, wherein the first notch of the acoustic waveguide is configured to receive the key.
TREATMENT OF ISCHAEMIA
An endovascular apparatus for crossing through an obstruction in a blood vessel comprises an elongate endovascular element such as a wire. The element has a proximal section, a distal tip section of smaller diameter than the proximal section; and a distally-tapering intermediate section extending between the proximal and distal tip sections. The apparatus comprises an ultrasonic transducer, mechanically coupled to the proximal section of the element for ultrasonically activating the element, hence exciting the distal tip section to facilitate crossing through the obstruction. A catheter surrounds the element, leaving at least part of the distal tip section of the element protruding distally beyond a distal end of the catheter.
Endovascular apparatus
An endovascular apparatus for crossing through an obstruction in a blood vessel comprises: an elongate endovascular element such as a wire; an ultrasonic transducer, mechanically coupled to the endovascular element for ultrasonically exciting a distal tip portion thereof to facilitate the crossing through the obstruction, and one or more damping features, mechanically coupled to the endovascular element to attenuate lateral displacement of the endovascular element at positions away from the distal tip portion.

An apparatus comprises a body assembly, a shaft, an acoustic waveguide, an articulation section, an end effector, and an articulation drive assembly. The shaft extends distally from the body assembly and defines a longitudinal axis. The acoustic waveguide comprises a flexible portion. The articulation section is coupled with the shaft. A portion of the articulation section encompasses the flexible portion of the waveguide. The articulation section comprises a plurality of body portions aligned along the longitudinal axis and a flexible locking member. The flexible locking member is operable to secure the body portions in relation to each other and in relation to the shaft. The end effector comprises an ultrasonic blade in acoustic communication with the waveguide. The articulation drive assembly is operable to drive articulation of the articulation section to thereby deflect the end effector from the longitudinal axis.
Systems and methods to modify intravascular lesions
An ultrasonic catheter assembly includes a sheath having a sheath lumen. A core wire is at least partially disposed within the sheath lumen. The core wire has a proximal portion and a distal portion. The proximal portion of the core wire is configured to be coupled to an ultrasound-producing mechanism. A working length of the distal portion of the core wire extends distally from the sheath. The working length is configured for longitudinal displacement, transverse displacement, or a combination of longitudinal and transverse displacement, in accordance with a plurality of output modes for vibrational energy supplied to the core wire proximal portion by the ultrasound-producing mechanism.
ULTRASONIC TREATMENT TOOL
An ultrasonic treatment tool includes: an ultrasonic oscillator in which a plurality of piezoelectric elements configured to generate ultrasonic oscillation are arranged along a longitudinal axis direction of the ultrasonic oscillator, the ultrasonic oscillator being configured to transmit the ultrasonic oscillation from a proximal end side to a distal end side of the ultrasonic oscillator; a contact receiver configured to transmit power and a control signal to the ultrasonic oscillator; and a floating structure configured to suppress transmission of ultrasonic oscillation generated by the ultrasonic oscillator to the contact receiver or dampen the ultrasonic oscillation.
Vibrating surgical instrument for liposuction and other body contouring applications
A vibrating hand held surgical instrument for loosening tissue of a patient for liposuction or body contouring procedures. The instrument includes a motor connected to a vibration actuator having an eccentric rotating mass and an end effector for engaging tissue operatively connected to the vibration actuator, wherein the motor rotates the eccentric mass to cause the end effector to vibrate to loosen tissue. A flexible shaft having first end and second ends dampen the vibration to the motor and to the operator handle.
ULTRASONIC CATHETER, DEVICE, AND SYSTEM
An ultrasonic catheter includes a catheter sheath, an ultrasonic transmission member, and a catheter body. The ultrasonic transmission member extends within a lumen of the catheter sheath. The catheter body is coupled to the catheter sheath. The catheter body has a fluid inlet port and an inner cavity in fluid communication therewith. A plurality of O-ring members is disposed around and contacts a proximal portion of the ultrasonic transmission member. An O-ring housing is located in the inner cavity of the catheter body, which is configured to contain the plurality of O-ring members. A perimetrical fluid channel is defined between an outer surface of the O-ring housing and the catheter body. The O-ring housing is configured such that a heat dissipating fluid flows into the perimetrical fluid channel and across the outer surface of the O-ring housing to dissipate heat from the plurality of O-ring members.
ROTATION FEATURES FOR ULTRASONIC SURGICAL INSTRUMENT
An apparatus includes a shaft assembly, an ultrasonic blade, and a clamp assembly. The shaft assembly includes an acoustic waveguide operable to transmit ultrasonic vibrations to the blade. The clamp assembly includes a clamp arm pivotable toward and away from the blade about a pivot axis, to clamp tissue between the clamp arm and the blade. A rotation feature may provide rotation of the blade relative to the clamp arm about the longitudinal axis of the waveguide. Alternatively, the rotation feature may provide rotation of the clamp arm relative to the blade about the longitudinal axis. The rotation feature may be driven based on pivotal positioning of the clamp arm relative to the blade about the pivot axis. The rotation feature may selectively lock and unlock the angular position of either the blade or the clamp arm about the longitudinal axis at any of a number of predetermined angular positions.
