An ultrasonic surgical instrument is disclosed including a handle including a pivoting handle portion, an end effector configured to treat tissue, a waveguide extending to the end effector, a battery and a vibration generating system selectively operable in a plurality of energizing operating states. The plurality of energizing operating states consists of a first operating state in which a first power is supplied to the end effector and a second operating state in which a second power is supplied to the end effector. The first power is different than the second power. The vibration generating system includes a transducer selectively attachable to the handle and a switch configured to operably switch the vibration generating system between the first operating state and the second operating state. The switch is further configured to operably switch the vibration generating system between the plurality of energizing operating states and a non-energizing operating state.

The catheter body has at least one lumen extending longitudinally therethrough. The elongate ultrasound transmission member extends longitudinally through the lumen. The elongate ultrasound transmission member has a distal portion and a proximal end configured to be coupled to an ultrasound transducer. The inflatable balloon, having an exterior surface, is coupled to the distal portion. The plurality of support wires are connected to the elongate ultrasound transmission member at two locations. A coating of at least one drug is on the exterior surface of the inflatable balloon. The elongate ultrasound transmission member is configured to transmit longitudinal vibrations to the inflatable balloon such that the inflatable balloon is longitudinally vibrated. The coating is removed by mechanical vibration of the inflatable balloon.

An endovascular apparatus for crossing through an obstruction in a blood vessel comprises an elongate endovascular element such as a wire. The element has a proximal section, a distal tip section of smaller diameter than the proximal section; and a distally-tapering intermediate section extending between the proximal and distal tip sections . The apparatus comprises an ultrasonic transducer, mechanically coupled to the proximal section of the element for ultrasonically activating the element, hence exciting the distal tip section to facilitate crossing through the obstruction. A catheter surrounds the element, leaving at least part of the distal tip section of the element protruding distally beyond a distal end of the catheter.

The clamp member pivotably movable relative to the ultrasonic blade between an open configuration and an approximated configuration with respect to the ultrasonic blade, wherein the clamp member is pivotably coupled to the inner tube, wherein the clamp member is pivotably coupled to the outer tube, and wherein movement of the outer tube relative to the inner tube between the first position and the second position transitions the clamp member between the open configuration and the approximate configuration.

An endovascular apparatus for crossing through an obstruction in a blood vessel comprises: an elongate endovascular element such as a wire; an ultrasonic transducer, mechanically coupled to the endovascular element for ultrasonically exciting a distal tip portion thereof to facilitate the crossing through the obstruction, and one or more damping features, mechanically coupled to the endovascular element to attenuate lateral displacement of the endovascular element at positions away from the distal tip portion.

The controller operatively connects to the ultrasonic blade and is configured to measure an ultrasonic frequency of the ultrasonic blade. The controller has a memory including a plurality of predetermined data correlations that correlate changes in measured ultrasonic frequency of the ultrasonic blade to a blade temperature of the ultrasonic blade.

An ultrasonic surgical instrument includes an end effector having an ultrasonic blade, an ultrasonic transducer assembly, and a shaft assembly the shaft assembly includes a tube, an acoustic waveguide, and a sheath radially positioned between the acoustic waveguide and the tube. The acoustic waveguide is received within the tube and is acoustically connected between the ultrasonic blade and ultrasonic transducer assembly to communicate ultrasonic vibrations from the ultrasonic transducer assembly to the ultrasonic blade. The acoustic waveguide extends along a longitudinal axis. The sheath includes a sheath body having an outer body surface and a plurality of damping rings radially extending from the outer body surface and engaged with the tube. The plurality of damping rings are configured to acoustically isolate the acoustic waveguide from the tube.
