Position detection based on tissue discrimination

A system is suggested comprising an optical sensing means and a processing unit. The optical sensing means may include an optical guide with a distal end, wherein the optical guide may be configured to be arranged in a device to be inserted into tissue in a region of interest. The processing unit may be configured to receive information of a region of interest including different tissue types as well as of a path through the tissues, to determine a sequence of tissue types along the path, to determine a tissue type at the distal end of the optical guide based on information received from the optical sensing means, to compare the determined tissue type with the tissue types on the path, to determine possible positions of the distal end of the optical guide on the path based on the comparison of tissue types, and to generate a signal indicative for the possible positions.

Methods of joint repair employing sutures and attached fixation devices are discussed. For example, a bone block graft procedure (e.g., Latarjet) is discussed which employs fixation devices to secure contact between graft surfaces of two bones. A suture construct, including a continuous suture loop routed through a first fastener, is secured to a first bone. Looped ends of the suture loop are passed through passageways formed in the two bones. The looped suture ends are further routed through a second fastener. The second fastener is mounted to the second bone and a sliding knot, formed in the looped suture ends, is advanced into contact with the second fastener. The suture is further tensioned using a tensioner device to secure the two bones together.

The implant is intended, in general, for surgical operations and, in particular, for interventional radiology.

A bone fastener driver assembly with a bone fastener retention feature has an outer sleeve, a handle fixed to said outer sleeve and an inner shaft. The inner shaft has a distal end with a torque driving tip. The inner shaft is coupled to the handle and axially movable relative to the outer sleeve from a proximal disengaged position to a distal engaged position. The outer sleeve has a bone fastener retaining distal end configured to lock into an internal groove or undercut of a bone fastener screw head when the inner shaft is axially moved from the proximal disengaged position to the distal engaged position to lock the bone fastener to the driver and upon a return movement of the inner shaft to the proximal disengaged position to unlock and release the driver from the bone fastener screw head.

An apparatus to limit a seating depth of a fastener can include a cylindrical housing, a pin, a driver, and a biasing element. The cylindrical housing can include a proximal portion couplable to a tool, a distal portion engageable with a workpiece, and a bore extending from an opening on the distal portion towards the proximal portion along a longitudinal axis of the housing. The pin can be secured to the housing and can extend radially inward into the bore. The driver can be translatable within the bore of the housing and can extend beyond the opening on the distal portion to engage the fastener. The driver can include a slot that can be engageable with the pin to receive a torque from the pin and can rotate with the housing to transfer the torque to a fastener when the pin engages the slot.
