Transcorneal and fiberoptic laser delivery systems and methods for the treatment of eye diseases wherein energy is delivered by wavelengths transparent to the cornea to effect target tissues in the eye for the control of intraocular pressure in diseases such as glaucoma by delivery systems both external to and within ocular tissues. External delivery may be affected under gonioscopic control. Internal delivery may be controlled endoscopically or fiber optically, both systems utilizing femtosecond laser energy to excise ocular tissue. The femtosecond light energy is delivered to the target tissues to be treated to effect precisely controlled photodisruption to enable portals for the outflow of aqueous fluid in the case of glaucoma in a manner which minimizes target tissue healing responses, inflammation and scarring.

The PI may include a closed, fluid-filled bladder having fiducials that contacts and deforms to the eye. Or, the PI may have an applanation lens with an outer ring portion and an inner concave portion for receiving the apex of the cornea. Another PI features a suction ring with a flexible skirt for contacting the sclera that is non-circular and/or non-planar. A system for injecting an index matching fluid into the area above the eye may also be incorporated. An integrated system includes a co-molded lens cone and attachment ring, with a lens window at the bottom of the lens cone which provides a sealed volume for vacuum-attaching a laser delivery system above the lens cone.

A patient interface device for an ophthalmic surgical laser system includes a lens cone component for coupling to the laser system and a suction ring component having a flexible skirt for coupling to the patient's eye. The suction ring component includes a gripper integrated with the flexible skirt, formed of hard plastic material and having two jaws that face each other to define a receiving opening between them. During use, a base portion of the lens cone is inserted into the receiving opening of the gripper and retained by the clamping force of the jaws. To ensure that the gripper securely retains the lens cone, a soft thermoplastic elastomer material is provided on the outer surface of the base portion of the lens cone, and the inner surfaces of the jaws of the gripper are provided with teeth to engage the soft thermoplastic elastomer material of the lens cone.
