Medical devices in accordance with various embodiments of the present invention employ one or more coaxial screw gear sleeve mechanisms. In various embodiments, coaxial screw gear sleeve mechanisms include a post with a threaded exterior surface and a corresponding sleeve configured to surround the post, the corresponding sleeve having a threaded interior surface configured to interface with the threaded exterior surface of the post and a geared exterior surface. A drive mechanism can be configured to interface with the geared exterior surface of the sleeve, causing the device to expand.
System and method for joint resurfacing and repair
An implant resection system for preparing an implant site to replace a defect in an articular surface of a first bone includes a first guide configured to be coupled generally to the first bone. The first guide includes a body portion defining a channel configured to receive a pin, wherein the pin is configured to penetrate and form a longitudinally disposed bore within the first bone. The implant resection system further includes a second guide configured to be coupled generally perpendicular to the first bone proximate to the defect by way of the bore. The second guide includes a drill bit configured to form an excision site through a portion of the articular surface in preparation of receipt of an implant.
Bone graft and methods of fabrication and use
Provided herein is a living bone graft including a biofabricated graft core including demineralized bone matrix and a carrier and a pre-vascularized shell at least partially enrobing the graft core, the pre-vascularized shell including isolated, intact adipose-derived microvessel fragments, mesenchymal stem cells, and collagen. The disclosed bone grafts include stromal cells that differentiate and microvessels that inosculate to provide a functional microvasculature, thereby approximating native bone repair as the graft matures in the patient. Also provided herein are methods of fabricating a bespoke, living, vascularized bone graft and methods of treating a segmental bone defect in a patient.

A cell-scaffold device includes at least one channel network including an inlet, a plurality of channels include a parent channel having an end portion communicating with the inlet and another end portion communicating with a first bifurcation, forming two child channels. Each child channel has an end portion communicating with a respective end portion of the first bifurcation and another end portion communicating with a second bifurcation, forming two grand-child channels from each child channel. Each grand-child channel has an end portion communicating with a respective end portion of the second bifurcation and another end portion. The other end portion of the grand-child channel either forms an outlet or a third child channel in communication with the grand-child channel. Each forming of grand-child channels defines a generation of the fractal structure. The devices are of use as scaffolds for seeding, growing, and maintaining cells implanted in and/or on the device.

The scaffold-implant may be machined into a custom-shaped three-dimensional cell culture system for support of cell growth, reservoir for peptides, recombinant growth factors, cytokines and antineoplastic drugs in the presence of a hyaluronan and/or dexamethasone alone or in combination with growth factors or transfected osteogenic genes, to be assembled ex vivo in a tissue incubator for implantation into bone tissue.

The invention relates to an artificial vascular graft comprising a primary scaffold structure encompassing an inner space of the artificial vascular graft, said primary scaffold structure having an inner surface facing towards said inner space and an outer surface facing away from said inner space, a coating on said inner surface, wherein a plurality of grooves is comprised in said coating of said inner surface. The primary scaffold structure comprises further a coating on said outer surface. The primary scaffold structure and the coating on said inner surface and on said outer surface are d designed in such a way that cells, in particular progenitor cells, can migrate from the periphery of said artificial vascular graft through said outer surface of said coating, said primary scaffold structure and said inner surface to said inner space, if the artificial vascular graft is used as intended. The invention relates further to a method for providing said graft.
Method for relieving back/neck pain and repairing intervertebral disc
A distal portion of a filament is extended beyond the distal end of a needle containing a gripper. A flexible one-way filament retainer with a snagging point is positioned adjacent to the extended filament. The needle with the extended filament and the one-way filament retainer are inserted into a cannula. During partial withdrawal of the needle, the snagging point of the one-way filament retainer hooks or retains the distal portion of the filament, depositing a section of the filament between the snagging point of the one-way filament retainer and the needle. When the needle is re-advanced, the needle pushes open the flexible one-way filament retainer, and the section of the filament is expelled or deposited in tissue. The needle can be rotated; the gripper engages and spirals the expelled filament to burrow into tissue. The method of needle partial withdrawal, re-advancement, rotation and pushing is repeated to pack and fill the tissue with interconnecting spirals of filament.
