Disclosed is an inter- and intra-catheter flow device for the management of vascular bleeding disorders that provide a liquid flow-pass between proximal and distal balloons for bridging the circulation between the upper and lower segments of a hemorrhaging artery or blood vessel, while blocking the blood flow to the hemorrhaging middle segment(s) of the artery or blood vessel between the two or more balloons. When only one balloon is inflated, these devices can create a pressure gradient between proximal or distal and middle segments of the artery or blood vessel. These devices are useful for controlling proximal artery blood pressure, preventing distal ischemia-reperfusion injury, identifying the bleeding location, controlling the bleeding, repairing and remodeling vascular structures, extending resuscitative endovascular balloon occlusion of the aorta (REBOA) use duration, and performing fluid resuscitation.

A system for perfusing a localized site within a body includes a catheter assembly having a venous access line that is adapted to deliver perfusate to the localized site, a venous or arterial drainage line adapted to drain perfusate from the localized site, and an occlusion device adapted to prevent some or substantially all physiological blood flow between the localized site and the systemic circulation of the body during and in the course of perfusing and draining perfusate to and from the localized site. The system may include a blood circuit associated with the catheter assembly to facilitate blood conditioning for use as the perfusate, in the course of a controlled perfusion and/or drainage of untreated, treated, or inactivated treated blood to and from the localized site. A delivery machine may control the blood circuit and catheter assembly in order to both deliver perfusate to, and drain some or all perfusate from, the localized site in a manner that provides perfusate to substantially only the localized site.

A method of delivering a therapeutic agent into a patient vessel with a delivery device is provided. The delivery device includes an infusion lumen through which the agent is delivered, a vessel occluder that can be operated between a collapsed and an expanded configuration, and when in the expanded configuration occludes the vessel, and a pressure sensor shielded from the effects of turbulent flow at an exit of the infusion lumen. The shield pressure sensor is able to accurately sense pressure in the vessel. The sensed pressure is used to determine a dwell time for the vessel occluder in the expanded configuration.

An innovative medical device that permits rapid, minimally invasive restoration of blood flow across a vascular blockage. A system allowing for lysis or removal of said blockage. Said device creates a temporary bypass using longitudinal structure configured for insertion into the blood vessel and adapted to deliver a side hole to a target area. The side hole defines a distal first segment and a proximal second segment with a lumen to allow blood flow therethrough to at least one distal end hole. Said device includes at least one semi-permeable membrane which may act as a filter that located circumferentially around outer surface of at least one of said device segments. In an alternate embodiment, a slidable outer sheath can cover the side hole to permit reversal of blood flow from the distal end hole to a proximal end hole located outside a patient's body by means of an aspiration controller. Alternate embodiments include an optional anchoring balloon, a macerating stent or wires, perforations for fluid delivery, and a backflow valve.

A system and methods are described for improving the management of ischemic cardiac tissue during acute coronary syndromes. The system combines a catheter-based sub-system which allows for simultaneous balloon dilation of a coronary artery and infusion of a carefully controlled perfusate during percutaneous coronary intervention. The system allows for modulation of levels of oxygen at the time of percutaneous intervention. In addition, catheters and systems are provided for administration of fluids with modified oxygen content during an intervention that incorporate upstream flow control members to compartmentalize the perfusion of the target coronary artery and the remainder of the heart.

A catheter-based system is provided having two occlusion elements and a perfusion lumen having a distal opening distal to the occlusion elements and a proximal opening proximal to the occlusion elements. The occlusion elements can be expanded to block blood flow both distal to an intravascular treatment site and to block blood flow proximal to the treatment site, thereby isolating the treatment site. The perfusion lumen can allow blood to flow between the distal and proximal openings, through the treatment site, while remaining isolated from fluids in the treatment site. The system can further include a treatment lumen having a proximal opening accessible by a user to provide aspiration, drug delivery, mechanical device delivery, or other treatment into the vasculature and a distal opening positioned between the two occlusion elements and in communication with fluids at the isolated treatment site.
