The present invention discloses a medical assembly including an elongate member having a proximal end portion and a distal end portion with a tapered tip. The tapered tip is configured to slide through a bodily tissue. The elongate member has a width referred to as a first width across at least a portion of the elongate member. The medical assembly further includes an implant having a first surface and a second surface. The implant is coupled to the elongate member such that a portion of the first surface of the implant is overlaid over a portion of the elongate member while the second surface faces opposite to the elongate member and is configured to contact the bodily tissue while being inserted. The implant has a width referred to as a second width such that the second width is smaller than the first width of the elongate member.

A minimally invasive surgical method of approximating tissue includes coupling an adjustable loop of suture around an inserter. The loop can be coupled to the inserter by applying tension to a tail of the suture to collapse the loop around the inserter, then the inserter and loop are passed through a first location in tissue. The loop is decoupled from the inserter, the inserter is retracted from the first location, and reinserted into a second location in the tissue, after which the loop is recoupled to the inserter while the loop is distal of the tissue. The decoupling, retraction, and reinsertion can occur without withdrawing the inserter from the patient's body. The inserter and loop are then withdrawn together from the tissue through this second location and outside of the patient's body. The tails and loop can form a luggage knot to be reduced around the tissue.

Arthroscopic meniscal repair systems and methods

An arthroscopic meniscal tear repair device includes a catch needle and a transfer needle, which are pierced into a torn meniscus and advanced past the tear. Suture is transferred by a suture needle from the transfer needle through the meniscus and into the catch needle. The catch needle has an internal mechanism that retains the suture. The suture needle is then retracted back to its home position inside the transfer needle, leaving the free end of the suture across the meniscus and in the catch needle. The device is then retracted out of the meniscus, leaving behind a stitch across the meniscal tear inside the meniscus. A pre-tied knot of suture is then slid down the device and cinched up using a knot pusher having a dilation tip, thus completing the repair.

A method and device for anchoring a length of self-retaining suture. The method of anchoring includes providing for an assembly having a length of self-retaining suture and a suture insertion device. The length of suture includes at least a portion of the length having a plurality of retainers thereon extending in a first direction. The insertion device has a length and a recess to receive a portion of the suture length. The method further includes placing a portion of the suture length in the recess and inserting the device into the body of a mammal until the recess reaches a predetermined location thereby forming an insertion pathway. The method further includes retrieving the insertion device from the body by moving the insertion device in a direction substantially opposed to the insertion pathway.

Angled suture passer and method of use thereof

A suture passer which is sized and shaped to be inserted through the femoral notch during a meniscal repair. The suture passer includes upper and lower jaws that may be straight or that may have an angled offset in relation to the common axis of the suture passer in order to gain an indirect access to meniscal tissue in a constricted space. The jaws of the suture passer may be angled, either to the left or to the right, up to 45 degrees from the central axis of the shaft. The suture passer is also able to grasp meniscal tissue prior to suture passing. A nitinol needle housed within one of the jaws can then be deployed to pass a preloaded suture through the tissue to be captured on the opposite side of the tissue.

A suture passer includes a first shaft comprising a hollow tube and a distal portion located distally of a distal end of the hollow tube; a needle comprising a needle tip and a needle shaft translatably mounted in the hollow tube for passing a suture loaded holder through tissue positioned between the distal end of the hollow tube and the distal portion of the first shaft; an actuatable engagement located in the distal portion of the first shaft for retaining the suture loaded holder after the suture loaded holder has passed through the tissue; a sheath extending around an exterior of the needle shaft and translatable with the needle shaft for at least a portion of a stroke of the needle, wherein the sheath is moveable relative to the needle for actuating the engagement to an unlatched position to release the holder from the distal portion of the first shaft.

An adjustable suture passer for use in arthroscopic surgery is disclosed. In a preferred embodiment, the adjustable suture passer comprises a head that describes a semicircular arc, an elongate body, a support element, and driving and control mechanisms. The head is adapted to accommodate a surgical needle and a guide wire. When the adjustable suture passer is activated, the needle is driven with sufficient force to penetrate bone. Use of the adjustable suture passer thus enables surgical attachment of soft tissue to bone without any necessity for a separate anchor.
