Disclosed is a suture delivery device for endoscopically passing a suture into biological tissue. The device can include two hypodermic needles that can be manipulated via controls on a handle of the device in order to pass a suture from one needle to the other. For example, the device can include a first needle coupled to at least one loop member that, when deployed from the first needle, provides a target through which a second needle of the device can deliver the suture. Once the second needle intersects the loop member, the user can advance a length of suture out through a distal end of the second needle. The loop member can then be retracted into the first needle, thereby grabbing and securing the suture in the process.

A reconfigurable suture needle includes an elongated body having a curved proximal section and a curved distal section. The curved proximal section is hollow and includes an elongated conduit extending from a proximal end to a distal end thereof. The curved distal section includes a needle point at a distal end thereof and a hollow post projecting proximally from a proximal end thereof. A suture thread passes through the elongated conduit of the curved proximal section and has a distal end that passes through the hollow post of the curved distal section for being secured to the proximal end of the curved distal section. The elongated body has a first configuration with a higher profile in which the hollow post of the curved distal section is inserted into the elongated conduit at the distal end of the curved proximal section, and a second configuration with a lower profile in which the hollow post of the curved distal section is uncoupled from the elongated conduit at the distal end of the curved proximal section.

The apparatus includes a needle including a proximal end including a hub, a distal end including a sharpened distal tip, a lumen having an oblong cross-section extending proximally from the distal end, and defining a longitudinal axis between the proximal and distal ends, and a clip deliverable from the lumen. The clip is compressible between a relaxed state in which a plurality of tines of the clip are shaped to engage and close a tubular structure within a patient's body, and a stressed state in which the tines are compressed to allow the clip to be loaded into the. The apparatus may also include a pusher member for deploying the clip from the distal tip of the needle such that the tines engage and close a tubular structure through which the tubular member is directed.

Instruments for delivering transfascial sutures and methods of transfascial suturing

An instrument for delivering a suture transfascially may include a handle, a shaft extending from the handle, and at least one needle that is moveable to an extended position beyond the end of the shaft. A suture may be delivered transfascially with the instrument. The instrument may include a suture catch associated with each needle for retaining and releasing a suture segment. A shield may be provided to shield the sharp end of each needle when it is moved to the extended position. A method of delivering a transfascial suture may include inserting an instrument into an abdominal cavity and deploying, from within the abdominal cavity, each needle through a soft tissue repair patch and then through at least part of the abdominal wall. A suture or suture segment may be advanced across the fascia with the at least one needle.

The suturing instrument uses a tissue suturing method consisting in introducing a quick-hardening biocompatible mass (hereinafter referred to as the ‘mass’) into the tissues to be connected or drawing it from them through hollow needles. The instrument has housing (1) with tissue gripping jaws (2 and 3) with control handles (5, 14). One of the jaws contains needle holder (19) with mechanism (17) moving it both ways. The needle holder contains mass receptacle (25, 28) above needles (18) that is connected to needle mouths via damper (33). A mechanism squeezing the mass out of the receptacle is located above the receptacle: inflatable bladder (29) that is open to external source of elevated pressure (31), rod or lock plate (59) with a device securing it to jaw walls (3) or instrument housing walls (1) on needle withdrawal from tissues. The mass receptacle presses against the fixed lock plate on needle lifting, and the mass is uniformly fed into tissues. The instrument can also be mounted on surgical forceps, (FIG. 1).
