A rongeur cutting system and method of operating said system. The rongeur cutting system may comprise a cutting unit that includes an outer tube and a cutting implement capable of both oscillation and longitudinal movement within the outer tube. The rongeur cutting system may also comprise a handpiece including a power source and spindle for actuating the cutting implement. The method of operating the rongeur cutting system may comprise operating the rongeur cutting system in a manual mode and/or operating the rongeur cutting system in a power mode. The manual mode includes moving the cutting implement longitudinally within the outer tube to manually cut tissue without utilizing the power source, and the power mode includes actuating a motor to manipulate the cutting implement within the outer tube while moving the cutting implement longitudinally within the cutting tube.

A vascular device is provided having a catheter body and a rotatable cutter assembly located at the distal end of the catheter body. The cutter assembly has at least one helical cutting surface within a housing that is coupled by a torque shaft to a drive mechanism. A conveyor mechanism helically wound about the torque shaft conveys occlusive material conveyed into the housing by the helical cutting blade further proximally along the catheter body for discharge without supplement of a vacuum pump. The catheter body is manipulated to insert the distal end of the catheter body within a body lumen and advance the distal end of the catheter body toward the occlusive material. The drive mechanism is operated to rotate the helical cutting surface to cut and convey the occlusive material from the body lumen proximally into the housing and to convey the occlusive material conveyed into the housing by the helical cutting surface further proximally along the catheter body by the conveyor mechanism for discharge without supplement of a vacuum pump. The distal end of the catheter body is deflected and rotated to sweep the cutter assembly in an arc about the center axis of the catheter body to cut occlusive material in a region larger than the outside diameter of the cutter assembly.

A distal housing is mounted on the distal end of the shaft and optionally includes a laterally open window where a plane of the window is generally perpendicular to the longitudinal axis of the shaft. An interior channel extends axially through the shaft and further through an interior of the housing to the window in the housing. An electrode member with a serrated or other elongated edge may extend longitudinally across the window and may be configured to reciprocate the elongated edge longitudinally relative to the window.

A method for removing materials from a subject can include inserting an endoscope into the subject through an esophagus of the subject; introducing, through a cavity wall of a stomach of the subject to access a site within the subject outside the stomach, an endoscopic tool coupled with the endoscope; actuating a cutting assembly of the endoscopic tool to cut material at the site, the material associated with at least one of a pancreatic fluid collection in a pancreas of the subject or an extra-pancreatic fluid collection external to the pancreas; and applying suction to a first end of an aspiration channel of the endoscopic tool to remove the material through the aspiration channel, the aspiration channel extending from the first end to a second end at an opening of the cutting assembly.

An endoscopic surgical instrument includes a main body assembly, and a cannula having a lumen and formed with a slot. An inner tube, which houses a spring-biased retractable cutting blade assembly having a cutting blade, is mounted on one end of the main body assembly and extends axially therefrom. The tube is receivable within the lumen of the cannula, and may be particularly oriented within the cannula lumen such that the cutting blade may be caused to project from both the tube and the cannula slot. The inner tube has a bore in which the distal end of an endoscope may be received. The instrument may be locked in one position to allow the endoscope to engage the blade assembly and cause the cutting blade to project from the cannula slot during a tissue cutting procedure, or may be locked in another position wherein the cutting blade remains retracted.

The surgical device further includes a tissue-removal assembly coupled to the second end of the shaft. The tissue-removal assembly includes a first cutting member having a plurality of rotatable blades. The first cutting member is coupled to a second end of the shaft. The tissue-removal assembly further includes a second cutting member, one or more support elements slidably or fixedly coupled to the second cutting member, and one or more extendable elements slidably or fixedly coupled to the second cutting member.
