Clot removal methods and devices with multiple independently controllable elements

A clot removal device for removal of an occlusion from a lumen in a patient's body is provided. The clot removal device has a lumen, an elongated member positioned within the lumen and extending axially from a proximal end to a distal end of the lumen, a handle attached to the proximal end of the lumen, a first expandable member positioned along a length of the elongated member, a second expandable member positioned along the length of the elongated member, wherein the second expandable member is distal to the first expandable member relative to the handle. The handle has at least one actuation mechanism and at least one of the following applies: a) the first expandable member is coupled to the at least one actuation mechanism and is configured to be moveable relative to the second expandable member upon manipulation of the at least one actuation mechanism; b) the first expandable member is configured to mechanically expand or contract by manipulating the at least one actuation mechanism; c) the second expandable member is coupled to the at least one actuation mechanism and is configured to be moveable relative to the first expandable member upon manipulation of the at least one actuation mechanism; or d) the second expandable member is configured to mechanically expand or contract by manipulating the at least one actuation mechanism.

A method for performing balloon catheterization of a zone of attention is provided. An intravascular device has an expandable portion secured to a distal end of a catheter tube. The expandable portion has a number of struts, at least one having an incising element extending perpendicular therefrom. The intravascular device is placed within a person's vascular system and moved to the zone of attention where the expandable portion is expanded and manipulated along the zone of attention to score the atherosclerotic material located in the blood vessel. Subsequently, a balloon catheter device is moved to the zone of attention and inflated to compress or fragment the scored atherosclerotic material.

Atherectomy medical device

Medical devices and methods for using medical devices are disclosed. A rotational atherectomy device may include an elongate shaft having a proximal end region, a distal end region and a lumen extending therein. The medical device may also include a cutting member positioned adjacent the distal end region of the elongate shaft. The cutting member may be radially expandable. The medical device may also include a sizing member positioned adjacent the cutting member. The sizing member may be radially expandable, and the radial expansion of the cutting member may be limited by radial expansion of the sizing member.

Method and apparatus for centering a microcatheter within a vasculature

The invention is directed to methods and apparatus for centering a microcatheter within a vasculature. In one aspect, the methods and apparatus can be used to support centering and to facilitate a guidewire to cross through a chronic total occlusion. In one embodiment, the catheter apparatus includes: a microcatheter having a lumen, a distal opening and a distal end; one or more guidewires for passing through the lumen of the microcatheter; an inner shaft having a lumen, a distal opening and a distal end for passing the inner shaft over the microcatheter; an outer shaft having a lumen, a distal opening and a distal end for passing the outer shaft over the inner shaft; and a self-expandable scaffold structure disposed towards the distal end of the inner shaft. The self-expandable scaffold structure is preferably non-occluding thereby allowing blood to flow through the scaffold. Preferably, the inner shaft and microcatheter are capable of being independently operable.

Tissue slitting methods and systems

Methods and systems for separating an object, such as a lead, from formed tissue are provided. Specifically, a tissue slitting device is configured to engage patient formed tissue at a slitting engagement point. While the object is subjected to a first traction force, the tissue slitting device is caused to move further into the engaged tissue and slit the tissue past the point of engagement. The slitting device causes the tissue to separate along an axial direction of the length of the formed tissue and releases at least some of the force containing the object. The methods and systems are well suited for use in cardiac pacing or defibrillator lead explant procedures.

THROMBUS RETRIEVER

The invention provides a thrombus retriever and associated methods for use. The thrombus retriever has a retriever body that is attached to a centrally-disposed deployment member capable of moving the retriever body in the proximal direction (towards the operator) in order to retrieve the target thrombus. The retriever body is attached to the deployment member at an attachment point distal to the most distal end of the retriever body. In use, pulling the deployment member in the proximal direction effectively pushes the retriever body from behind during thrombus collection. The thrombus retriever also may comprise a net at its distal end to collect the retrieved thrombus and prevent embolization.
