[Solution] A catheter 10 has a shaft 11 having an opening 20 in a part of the side wall on the distal end side, a cutter 12 which is located in the vicinity of the opening 20 in the internal space of the shaft 11 and which can move in the axial direction 101 of the shaft 11, a balloon 23 which is disposed on the side opposite to the opening 20 with respect to the axis of the shaft 11 and which outwardly expands from the side wall of the shaft 11, and a phased array ultrasound probe 17 disposed along the circumferential direction 102 of the outer peripheral surface of the side wall in the vicinity of the opening 20 at least on the same side as the side where the opening 20 is provided with respect to the axis of the shaft 11.
Surgical systems and tools for moving energy applicators in superimposed modes
A surgical tool may move a saw blade in a sagittal plane. The saw blade has a distal blade end with teeth. The surgical tool comprises a handpiece body and a carrier operatively attached to the handpiece body. The carrier is configured for oscillating movement about a pivot axis. A retainer is operatively attached to the carrier for concurrent movement and releasably secures the saw blade in the sagittal plane relative to the handpiece body. An actuator is coupled to the handpiece body to selectively oscillate the carrier relative to the handpiece body such that the retainer and the saw blade pivot back and forth about the pivot axis within the sagittal plane. An ultrasonic transducer is operatively attached to the handpiece body to selectively generate ultrasonic energy to resonate the saw blade such that the teeth at the distal blade end reciprocate proximally and distally within the sagittal plane.

Echolucent Intravascular Cannula and Echolucent Locating of Portions of Intravascular Medical Devices
A medical tube or cannula comprising enhanced imaging structure and/or materials is provided. In some embodiments, an otherwise solid echogenic band may be interrupted by echolucent features and/or materials. In other embodiments, an echogenic band may be adjacent to an echolucent band, while in other embodiments one or more echolucent bands may be provided. In some cases, two or more spaced-apart echolucent bands may be provided. In some embodiments, an echolucent band may comprise an echogenic feature or materials. Generally, the juxtaposition of echogenic and echolucent materials enhances the imaging contrast of an intravascular device and allows easy identification and positioning of the juxtaposed echogenic and echolucent regions.
HYBRID ATHERECTOMY DEVICES
Telescoping, self-driving, and laterally-pushing atherectomy devices are provided, each having a flexible sheath, a cutter with helical flutes, and a drive assembly. The drive assembly can have a flexible driveshaft rotatably translational with the lumen of the flexible sheath, a positive displacement pump to transport cut tissue, and a flexible drive shaft that can be longer than the flexible sheath for a reversible telescoping of the drive assembly from the lumen of the flexible sheath. The positive displacement pump can be a screw pump having a drive screw portion exposed for contact with a vascular lumen for a self-driving of the device through the vascular lumen. A reversibly-expandable, lateral pushing member can be included at the distal end of the flexible sheath for a lateral pushing of the cutter. Improved cutting heads, and methods of making them, are provided for cutting a combination of soft and hard plaque.

A gearless rotational motor comprising a motor drive shaft adapted to be rotated by the gearless rotational motor, the motor drive shaft having a lumen, a proximal end extending proximally from the motor and a distal end extending distally away from the motor. The distal end adapted to connect with a torqueable flexible shaft whereby rotation of the motor drive shaft also rotates the torqueable flexible shaft.

METHODS FOR CROSSING AND TREATING AN OCCLUSION
A device and method for treating a patient with total or near total occlusion is provided. The device can be positioned in a blood vessel at a treatment site. An occlusion at the treatment site is enlarged by a catheter. The catheter can be advanced over a guidewire into the occlusion. One or more of [a] compression or torsion applied to the guidewire or [b] compression or torsion applied to the catheter body expands or creates a path through the occlusion. The expansions or creation of the access path can be by cutting or abrading the occlusion or by a shoe-horn effect.
