Method and apparatus for minimally invasive insertion of intervertebral implants

A dilation introducer for orthopedic surgery is provided for minimally invasive access for insertion of an intervertebral implant. The dilation introducer may be used to provide an access position through Kambin's triangle from a posterolateral approach. A first dilator tube with a first longitudinal axis is provided. A second dilator tube may be introduced over the first, advanced along a second longitudinal axis parallel to but offset from the first. A third dilator tube be introduced over the second, advanced along a third longitudinal axis parallel to but offset from both the first and the second. An access cannula may be introduced over the third dilator tube. With the first, second, and third dilator tubes removed, surgical instruments may pass through the access cannula to operate on an intervertebral disc and/or insert an intervertebral implant.

A system for performing surgical repair of the spine includes a distractor and a permanently implanted bone plate system. A surgical repair methodology is also disclosed that employs an implanted bone plate system with a substantially void internal volume which is attached to adjacent vertebrae subsequent to the distraction and adjustment of curvature of the vertebrae and prior to the excision of disc and/or end plate tissue through the bone plate. The device further facilitates the subsequent delivery of an interbody repair device for the purpose of either fusion or dynamic stabilization, such as by disc arthroplasty. The plate may be permanently implanted, such as when a fusion between the attached vertebral bodies is desired, but it need not be permanently implanted.

The present invention relates to a series of orthopedic plates for use in repair of a bone. The plate has a Y-shaped profile or an X-shaped profile which includes an elongate central trunk with a complex contour and either one or two terminal pairs of arms that have a first arm and a second arm that form differing angles and lengths relative to the trunk portion of the plate. The arms include locking screw holes where the screws converge toward each other to provide for multiplanar fixation but which do not impinge.

Methods of implanting a prosthesis to repair a joint include displacing a first bone from the joint formed by an intersection between the first bone and a second bone. An end portion of the first bone is resected to define a resected end. A concavity is formed into the resected end using a shaping tool. The bone is compacted to form a support layer lining the concavity. The prosthesis is implanted in the concavity against the support layer without attaching the prosthesis to the support layer. The joint is reformed with the prosthesis such that the prosthesis remains unattached to the support layer and the first and second bones articulate about the prosthesis.

A tool set for preparing a joint, inserting an implant or removing an implant from a joint in an open or less invasive procedure generally comprises a set of nested tools, pin guides, jigs, and/or immobilization elements. The nested tool set comprises at least one pin and at least one cannulated tool. The cannula can comprise tangs that project from the distal end of the cannula. The tangs can be offset such that drilling removes different amounts of bone relative to drilling through a centered drill guide. The cannulated tool kit can comprise at least one tool guide/cannula, drill bits/reamer, syringe, and/or inserter. At least one cannula is positioned over and around the pin position into the joint. The channel of the cannula guides the other tools into the joint.

Intra joint stabilization construct

The disclosure provides devices and methods of use pertaining to intra joint stabilization. Embodiments include a number of suture returning and suture locking anchors that feature elongated sleeves configured to protect the associated bone tunnels from suture wipering, which results in abrasion and enlargement of the bone tunnel and leads to migration of the suture anchors. Embodiments also include suture locking anchors that lock via an interference fit between the suture strand and a receiver of the anchor and a set screw, where the receiver and the set screw each feature a number of gradual, opposing tapers to facilitate gradual proximal-to-distal gripping and releasing of the suture strand to achieve an optimal locking force while preventing severing of the suture strand. Further embodiments include intra joint reinforcement and stabilization constructs formed using the disclosed devices. Other embodiments are disclosed.
