A system, apparatus, and method of using the same are disclosed for carrying out a transvascular procedure such as a Transjugular Intrahepatic Portosystemic Shunt (TIPS) procedure using energy to create a tract between the hepatic vein and the portal vein. The present invention provides a method using a flexible radiofrequency (RF) catheter having a lumen therethrough. The method involves the steps of advancing the flexible RF catheter into a first vessel; advancing a guiding sheath overtop of the flexible RF catheter; steering the guiding sheath to direct a distal tip of the RF catheter towards a second vessel; and, delivering RF energy via the flexible RF catheter, forming a tract from the first vessel and entering the second vessel.

Vessel sealing and cutting system by means of extra-luminal laser

The present invention is a vessel sealing and cutting system (10) comprising a surgical equipment (20) comprising a lower jaw (222) and an upper jaw (212) in a manner defining a holder tip (24) grabbing the tissues containing vessels, and a lower body (21) embodied at the continuation of said upper jaw (212), and a laser source (50) connected to said surgical equipment (20), characterized by comprising a module housing (215) embodied in a manner extending inside said lower body (21) and said upper jaw (212), and a laser module (30) which transmits a laser light to the grabbing region (61) from said holder tip (24) in a simultaneous manner with the closing of the jaws and positioned in said module housing (215) in a manner connected to said laser source (50) from one end thereof.

Sealing and/or cutting instrument

A sealing and/or cutting instrument having a thermally active surface or element which may be used to seal and then cut tissue, ducts, vessels, etc., apart. The instrument may include a thermally active surface or element comprised of a conductor covered with a ferromagnetic material. The instrument may contact tissue with one or more surfaces comprised of a non-stick material. A sensor in communication with the instrument may be used to monitor a therapeutic procedure and signal when sealing and/or cutting of a tissue is complete.

A surgical instrument includes a hollow member having a sidewall provided with a window and a closure member movably connected to the hollow member for alternately covering and uncovering the window. The hollow member has a first clamping surface along an edge of the window, while the closure member has a second clamping surface opposing the first clamping surface and disposable substantially adjacent thereto in a clamping or closure configuration of the instrument. The instrument additionally comprises a tissue occlusion component mounted to at least one of the hollow member and the closure member for acting on tissues gripped between the first clamping surface and the second clamping surface, to couple the tissues to each other.

ENDOSCOPIC VESSEL HARVESTER WITH INTEGRATED SUCTION LINE

An endoscopic vessel harvester includes a vessel keeper with a capture frame capturing a target vessel within a tunnel that has been dissected along the target vessel. A cutter member cuts and cauterizes side branches while the target vessel is slidably captured in the vessel keeper. A fixed supporting rod and a movable side rod engage the frame. The movable side rod has an opened position to admit the target vessel and has a closed position to slidably capture the target vessel. One of the fixed support rod or the movable side rod is comprised of a hollow cylinder open at the distal end and at the proximal end, and wherein the distal end of the hollow cylinder fluidically communicates with the tunnel through the capture frame. A suction tube connects to the proximal end of the hollow cylinder, allowing pooled fluid around the capture frame to be removed.

A scissor-style vessel sealing having a knife engagement mechanism that will automatically drive a knife between the jaws when the handles are moved into the closed position. The vessel sealer has a first shaft with a first jaw and a second shaft having a second jaw pivotally coupled to the first shaft so that the jaws are moveable between open and closed positions. A knife actuating assembly is coupled to the first shaft to move between a first position, where the knife is in a retracted position, to a second position, where the knife is in an extended position, in response to contact by the second shaft when the scissor-style vessel sealer is closed. A knife lockout may be used to disengage the knife engagement mechanism from the knife to prevent movement of the knife when the scissor-style vessel sealer is closed.
