An arthroscopic cutting probe includes an outer sleeve having a longitudinal bore and an outer cutting window at its distal end. The outer sleeve has a metal body electrically insulated over its exterior except for a portion at least partially surrounding the outer cutting window which comprises an active electrode. An inner sleeve is rotationally disposed in a bore of the outer sleeve, and the inner sleeve has a distal end, a proximal end and a longitudinal passageway having an electrically conductive inner surface which comprises a return electrode. A dielectric cutting member is attached to the distal end of the inner sleeve and has a central passage extending from an inner cutting window at its distal end to an opening at its proximal end. The opening at the proximal end is contiguous with the longitudinal passageway in the inner sleeve, and rotation of the inner sleeve relative to the outer sleeve causes the inner cutting window to rotate past the outer cutting window to resect tissue received through the cutting windows as they pass each other.

The output tip includes an electrode and insulating material as an outer layer.

The present disclosure is directed to an electrosurgical apparatus for generating plasma in electrosurgical applications. The electrosurgical apparatus includes an end effector disposed on a distal portion of a tube of the electrosurgical apparatus. The end effector mixes ambient air with an inert gas to increase the production of radical species. In one aspect of the present disclosure, the end effector includes a cylindrical augmenter disposed over a distal end of the tube with one or more tilted vanes disposed between the cylindrical augmenter and the tube. In another aspect of the present disclosure, the end effector includes one or more tilted vanes disposed on an inner surface of a wall of the distal end of the tube. In another aspect of the present disclosure, the end effector includes one or more advection apertures on the wall of the distal end of the tube.

A method in accordance with the present disclosure includes providing electrical treatment energy to an instrument based on an indicated electrical energy level, accessing voltage signal values over time relating to voltage of the electrical treatment energy and/or current signal values over time relating to current of the electrical treatment energy, determining whether an arc generated by the instrument is an arc to be maintained or an arc to be extinguished based on a threshold value and at least one of the voltage signal values or the current signal values, and controlling the electrical treatment energy based on determining that the arc is an arc to be extinguished.

A surgical fluid management system delivers fluid for distending a uterine cavity to allow cutting and extraction of uterine fibroid tissue, polyps and other abnormal uterine tissue. The system comprises a fluid source, fluid deliver lines, one or more pumps, and a filter for re-circulating the distension fluid between the source and the uterine cavity. A controller can monitor fluid retention by the patient.

An electrosurgical apparatus and method for cutting and resecting a tissue volume is described. The apparatus includes one or more electrosurgical filaments and capture components deployable by forward extension at or near a forward tip of an elongated shaft of the apparatus to form an elliptical path that defines, with the other capture components, a spheroidal receptacle configured to surround a tissue volume for resection. The elliptical path is formable by a first extension followed by a second extension, said first extension having a first axial component and a radial expansion component, and said second extension having a second axial component and a radial contraction component, where at least one of the capture components is of varying stiffness.

An electrosurgical resection instrument for applying to biological tissue radiofrequency electromagnetic energy has a protective hull comprising a shaped piece of dielectric material mounted to cover an underside of an instrument tip of the instrument. The protective hull acts as a shield to protect tissue that may lie under the instrument tip from damage during treatment. The instrument may be particularly useful in procedures performed in a gastrointestinal tract, where bowel perforation concern, or in the pancreas, where damage to the portal vein or the pancreatic duct may occur when a tumor or other abnormality is being resected, dissected or removed.
