Catheters, catheter systems, and methods for puncturing through a tissue structure and ablating a tissue region

A percutaneous catheter system for use within the human body and an ablation catheter for ablating a selected tissue region within the body of a subject. The percutaneous catheter system can include two catheters that are operatively coupled to one another by magnetic coupling through a tissue structure. The ablation catheter can include electrodes positioned within a central portion. The ablation catheter is positioned such that the central portion of a flexible shaft at least partially surrounds the selected tissue region. Each electrode of the ablation catheter can be activated independently to apply ablative energy to the selected tissue region. The ablation catheter can employ high impedance structures to change the current density at specific points. Methods of puncturing through a tissue structure using the percutaneous catheter system are disclosed. Also disclosed are methods for ablating a selected tissue region using the ablation catheter.

Tumors can be treated with an alternating electric field. The size of cells in the tumor is determined prior to the start of treatment by, for example, biopsy or by inverse electric impedance tomography. A treatment frequency is chosen based on the determined cell size. The cell size can be determined during the course of treatment and the treatment frequency is adjusted to reflect changes in the cell size. A suitable apparatus for this purpose includes a device for measuring the tumor impedance, an AC signal generator with a controllable output frequency, a processor for estimating the size of tumor cells and setting the frequency of the AC signal generator based thereon, and at least one pair of electrodes operatively connected to the AC signal generator such that an alternating electric field is applied to the tumor.

An electrosurgical resection instrument for applying to biological tissue radiofrequency electromagnetic energy has a protective hull comprising a shaped piece of dielectric material mounted to cover an underside of an instrument tip of the instrument. The protective hull acts as a shield to protect tissue that may lie under the instrument tip from damage during treatment. The instrument may be particularly useful in procedures performed in a gastrointestinal tract, where bowel perforation is a concern, or in the pancreas, where damage to the portal vein or the pancreatic duct may occur when a tumor or other abnormality is being resected, dissected or removed.

The second electrode is configured to move from the first position to the second position when a force is applied to the end effector by a tissue section. The first electrode and the second electrode define a treatment area when the second electrode is in the second position.

The device further comprises an RF generator arranged to supply RF energy to the user's skin via the active electrode and the return electrode. The return electrode has a planar skin contact surface extending in a main plane. The active electrode has a skin contact surface with a maximum dimension in a range from 100 μm to 500 μm, and a surface area of the planar skin contact surface of the return electrode is at least 5 times larger than a surface area of the skin contact surface of the active electrode. The skin contact surface of the active electrode is arranged in a position at a distance from the main plane, seen in a direction perpendicular to the main plane. The device may be advantageously used, for example, to control the dimensions and shape of a thermal lesion in the user's skin generated by the RF energy.

An electrosurgical scalpel includes a handle with a receptacle portion of a conductive hollow member for mounting and retaining an electrode blade and evacuating fumes, an optional light source with a power source that may be encapsulated within the handle and a vacuum port for applying a vacuum source to draw fumes away from the electrode tip through the chanel. Operation of a slide on the handle serves to retract or extend the electrode blade, and direct fume evacuation.
