Electrosurgical generator and methods

The disclosed technology is directed to a RF power generator and feedback control system used to regulate the electrical power delivered to a cutting filament (i.e., a cutting electrode) of an electrosurgical instrument. The electrosurgical instrument uses the delivered energy to form a cutting arc for ablating a tissue mass to access a target tissue therein. The instrument forms a basket-like receptacle around the target tissue to excise the target tissue from the ablated tissue mass. As the instrument forms the receptacle, the length of exposed filament ablating the tissue changes. To this end, the RF power generator described herein is configured to vary the total power delivered during the deployment of the instrument based on a measurement of output power derived from a differential phase angle between a current sense output and a voltage sense output, in some embodiments, to maintain a uniform power density along the length of exposed filament.

Fractional resection is applied as a stand-alone procedure in anatomical areas that are off-limits to conventional plastic surgery due to the poor tradeoff between the visibility of the incisional scar and amount of enhancement obtained. Fractional resection is also applied as an adjunct to established plastic surgery procedures such as liposuction, and is employed to significantly reduce the length of incisions required for a particular application. The shortening of incisions has application in both the aesthetic and reconstructive realms of plastic surgery.

An electrode assembly is provided. The electrode assembly includes a proximal end that is adapted to connect to an electrosurgical instrument including a housing defining a longitudinal axis therethrough and an electrosurgical energy source. A distal end includes a cutting electrode having a loop configuration configured to cut tissue. The distal end includes a return electrode operably disposed adjacent the cutting electrode. A dielectric shield is operably disposed between the cutting electrode and return electrode. The dielectric shield extending distally past the cutting electrode to hinder current flow to the return electrode when the dielectric shield, cutting electrode and return electrode are submersed in a conductive solution and the cutting electrode is energized, thereby concentrating current density at the cutting electrode.

A device for treating a surface with a dielectric harrier plasma, wherein the surface functions as a counterelectrode, has an electrically insulating housing and a housing head. A rotatable electrode connected to a high-voltage feed line is shielded by a dielectric and protrudes out of the housing head. The configuration has fundamentally unrestricted movement of the electrode over the surface to be treated in that the electrode has rotary bearing elements which are arranged in a center axis and a rotation axis is at an angle to the center axis.

ELECTROSURGICAL DEVICE FOR CUTTING TISSUE

A tool assembly for use with an electrosurgical device for cutting tissue includes a base portion, a return lead, an electrical insulator, a center pin, and an active lead. The center pin extends from the base portion and through a lumen of the electrical insulator. The active lead is securely fixed to the base portion and extends between the base portion and a distal portion of the center pin such that a portion of the active lead extends around the distal portion of the center pin and first and second segments of the active lead are spaced apart from the return lead. Upon activation, electrosurgical energy is transmitted from the active lead through tissue to the return lead to cut tissue in contact with the active lead.
