A method of forming and implanting a synthetic corneal lenslet in an eye of a patient includes the steps of: forming a synthetic lenslet from a collagen solution using a mold or a 3-D printer that are configured to form the synthetic lenslet into a predetermined shape for correcting a particular refractive error of the patient; forming a cavity for receiving the synthetic lenslet in the cornea of the eye of the patient; inserting the synthetic lenslet into the cavity of the eye; applying a photosensitizer into the cavity of the eye so that the photosensitizer permeates at least a portion of the tissue surrounding the cavity and at least a portion of the synthetic lenslet; and irradiating the cornea so as to activate cross-linkers in the synthetic lenslet and cross-linkers in the portion of the tissue surrounding the cavity, and thereby prevent an immune response.

A system for ophthalmic surgery includes a laser source configured to deliver an ultraviolet laser beam comprising laser pulses having a wavelength between 320 nm and 370 nm to photodecompose one or more intraocular targets within the eye with chromophore absorbance. The pulse energy, the pulse duration, and the focal spot are such that an irradiance at the focal spot is sufficient to photodecompose the one or more intraocular targets without exceeding a threshold of formation of a plasma and an associated cavitation event. An optical system operatively coupled to the laser source and configured to focus the ultraviolet laser beam to a focal spot and direct the focal spot in a pattern into the one or more intraocular targets. The optical system focuses the laser beam at a numerical aperture that provides for the focal spot to be scanned over a scan range of 6 mm to 10 mm.

The lenticules are cut according to a pattern that to maximizes the number of lenticules, thereby maximizing the number of implants from the single donor cornea. An example implant handling device includes a body. The body includes a flattened end configured to receive a corneal implant and keep the corneal implant from rolling or folding. The flattened end has a width and a height, the width being greater than the height. The body includes a slit opening to the flattened end, the slit opening configured to allow the corneal implant to pass into the flattened end.

The method includes forming a pocket in a cornea of an eye, applying a photosensitizer inside the pocket so that the photosensitizer permeates at least a portion of the tissue bounding the pocket, the photosensitizer facilitating cross-linking of the tissue bounding the pocket; irradiating the cornea so as to activate cross-linkers in the portion of the tissue bounding the pocket, and thereby kill cells therein; inserting a lens implant into the pocket; and applying laser energy to the lens implant in the pocket using a laser so as to correct refractive errors of the lens implant and/or the eye in a non-invasive manner. In other embodiments, a lens implant is soaked in a cross-linking solution that includes a photosensitizer prior to being inserted into the pocket in the cornea of an eye.

A method of corneal implantation with cross-linking is disclosed herein. In one or more embodiments, the method includes the steps of: (i) cutting a circular implant from a donor cornea; (ii) cutting the circular implant into a plurality of constituent pieces; (iii) forming a small incision in a recipient cornea of an eye of a patient; (iv) forming a pocket in the recipient cornea of the eye of the patient, the pocket being accessible through the small incision in the recipient cornea, and the pocket being bounded entirely by stromal tissue of the cornea; and (v) inserting each of the plurality of constituent pieces of the implant into the pocket of the recipient cornea via the small incision, wherein the implant is cross-linked so as to prevent an immune response to the implant and/or rejection of the implant by the patient.

The controller controls the cutting apparatus to direct, via the one or more optical elements, the laser from the laser source to sculpt the lenticule according to the sculpting plan to produce the corneal implant with the refractive profile.
