In an embodiment, an ophthalmic surgical laser system comprises a laser source generating a pulsed laser beam and a laser delivery system delivering the pulsed laser beam to a cornea of an eye. A patient interface couples to and constrains the eye relative to the laser delivery system. A controller controls the laser delivery system to perform an anterior surface volume dissection on the cornea.

The controller generates a sculpting plan for modifying a first shape of a lenticule formed from corneal tissue and achieving a second shape for the lenticule to produce a corneal implant with a refractive profile to reshape a recipient eye. The sculpting plan is determined from measurements relating to the lenticule having the first shape and information relating to a refractive profile for a corneal implant. The controller controls the cutting apparatus to direct, via the one or more optical elements, the laser from the laser source to sculpt the lenticule according to the sculpting plan to produce the corneal implant with the refractive profile.

A method for determining spherical aberration parameters for a corneal laser treatment to treat presbyopia may include performing pupillometry to measure various actual pupil diameters of a patient from a photopic diameter to a mesopic diameter. An actual pupil center of the patient may also be measured. The measured pupil diameters and the pupil center may be used to customize the spherical aberration parameters to the eye of the patient for improved ocular results after treatment.

A laser delivery system is configured to delivery light energy to soften and realign the tissue of the eye in order to increase accommodation and treat glaucoma. The laser system can be configured to increase a circumlental space of the eye and increase movement of a posterior vitreous zonule in order to increase accommodation. The light energy may comprise wavelengths strongly absorbed by collagen of the sclera. In many embodiments a heat sink is provided to couple to the conjunctiva and the heat sink comprises a material transmissive to the light energy absorbed by collagen, for example Zinc Selenide. The heat sink can be chilled to inhibit damage to the conjunctiva of the eye. In many embodiments, one or more layers of the epithelium of the eye remain substantially intact above the zone where the eye has been treated when the heat sink has been removed.
