A syringe assembly comprising a barrel for containing one or more medicaments, the barrel having a front outlet for allowing the expulsion of said one or more medicaments from the said barrel through said outlet, and a first stopper disposed in the barrel and axially moveable therein and defining a first volume in the barrel axially forwards of the first stopper and a second volume in the barrel axially rearwards of the first stopper. The first stopper has a two way valve that is moveable between a sealing configuration and an open configuration to selectively put the first volume in fluid communication with the second volume. The two way valve is moveable from the sealing configuration to the open configuration by an increase in fluid pressure in either of the first and second volumes. The syringe assembly additionally comprises a second stopper disposed in the barrel axially rearward of the first stopper and axially moveable therein, the second stopper providing a fluid tight seal at a rear end of the second volume. The syringe assembly further comprises a first plunger rod connected to the first stopper and extending axially rearwardly through the second stopper, the second stopper sealingly engaging with the first plunger rod but permitting axial sliding of the first plunger rod therethrough. The first plunger rod is axially moveable so as to axially move the first stopper and pressurise either of the first volume and second volume causing the two way valve to move into the open configuration and fluidly connect the first volume and second volume.

The present invention involves a sterilizable syringe allows for reduction in needlestick injuries using conventional sterile needles. The syringe allows for direct access to and the safe removal and disposal of needles. The device consists of a sterilizable, reusable hypodermic syringe to which a disposable needle is attached, and employs carpules containing injectable medication, viewable and either manually or automatically aspirating. Some embodiments have a protective sliding mechanism consisting of an open frame bearing one or more protective collars, one at the furthermost end of the frame and an optional second one at a more proximal position. The protective mechanism may be extended to protect an exposed needle tip and retracted during an injection. With the protective mechanism extended, the needle mounting hub is exposed to allow for its exchange or removal and safe disposal. The protective mechanism is engaged until immediately prior to the delivery of medication, disengaged on delivery, and engaged after delivery.

Optionally, a device of the present invention is a dual-mode device configured for being operated independently in each of an ejection mode and an injection mode. A cartridge for dispensing a pre-measured buffering agent together with a medical fluid such as dental anesthetic. The cartridge includes a sharp piercing element at one end, a frangible barrier proximate the piercing element, and a traveling closure. The buffering solution is in a chamber between the barrier and the closure. When loaded into a syringe and subjected to operating pressures, the barrier is pierced, enabling the buffering solution to mix with the medical fluid. Continued pressure propels a desired, controlled mixture into a tissue of a patient. A dye may be included for visual confirmation of mixing. The invention may be regarded as the cartridge, a syringe having the cartridge, or a kit, with or without a syringe, or cartridges having closures of different types.
