Method and device for the adaptive regulation of a positive end-expiratory pressure (PEEP)

A method controls an expiratory gas flow at a user interface (16) of a ventilator (1) wherein the user interface (16) has an exhalation valve (11), which provides a positive end-expiratory pressure (PEEP). The method includes the following steps during a phase of exhalation: changing the positive end-expiratory pressure from a basic PEEP value (31) with the exhalation valve (11); returning the positive end-expiratory pressure to the basic PEEP value (31) with the exhalation valve (11); and determining an exhalation parameter. The method permits an adaptive change in the expiratory flow during the exhalation. Air trapping can be avoided, and it is possible to respond to changed exhalation parameters within one and the same phase of exhalation.

The add-on device senses moisture at a distal end of the add-on device, and in case of detection of excessive wetness, provides suction to the distal end of the device through a cavity passing along the device, the suction is provided via a suction outlet channel extending from a proximal end of the device.

System and method for controlling and monitoring flow in an endotracheal tube

A system for controlling and monitoring flow in a cuffed endotracheal tube device is disclosed. The system comprises: a connector panel having at least three connectors adapted for establishing fluid communication with proximal ends of at least a first fluid line, a second fluid line and a cuff inflation line of the endotracheal tube device. The system can further comprise a processing unit and a control unit, wherein the processing unit is configured to instruct the control unit to execute various operations, including at least a rinsing procedure, a suctioning procedure, a cuff inflation procedure, a leak detection procedure and a venting procedure, and to select any of the first and the second fluid lines for any of the rinsing, suctioning, leak detection and venting procedures. In some embodiments, the system exploits the cuff as sensor to sense pulmonary data.

The measuring device is connectable to an endotracheal tube associated with a cuff inflatable below the vocal cords of a subject, for measuring at least one measure being indicative of the presence of leakage of secretions from above the cuff to the lungs of the subject. The controller adjusts inflation of the cuff responsively to the level of the at least one measure such as to reduce or prevent leakage of secretions from above the cuff to the lungs, and alerts that the endotracheal tube is malpositioned in the airway of the subject if an inflation pressure of the cuff exceeds a predetermined threshold.

An algorithm uses end-tidal carbon dioxide (EtCO.sub.2), and tidal volume (TV) or peak inspiratory pressure (PIP) to detect exact intubation time. Another algorithm uses respiratory parameters to identify and/or confirm the type of airway device used during mechanical ventilation, and to detect if and when an issue has arisen with use of a specific airway device to provide real-time decision support to attending medical care professionals.
