A two-piece suspension pedicle anchor and its system and component-set is disclosed. The two-piece suspension pedicle anchor comprises a proximal piece and a distal piece that can rotate relatively. An external thread is arranged on the outer surface of the proximal piece, and a transverse through hole for an artificial ligament to pass through is arranged at the distal piece. As the two-piece suspension pedicle anchor is rotated and gradually locked into the bone, only the proximal piece is rotated, and the distal piece with the artificial ligament is pushed down but not rotated; in addition, the artificial ligament will be brought into the bone along with the distal piece and sandwiched between the outer surface of the proximal piece and the bone, that is, the artificial ligament will be sandwiched and fixed between the anchor-bone interface without wrapping around the outer surface of the proximal piece.

Surgical instrument and method

A surgical instrument comprises a first member including a lateral projection having a locking surface that defines an elongated cavity. The locking surface is engageable with a longitudinal member to fix the longitudinal member with the first member. A second member includes at least one mating element being engageable with a spinal construct. The first member is axially translatable relative to the second member in a first direction to tension the longitudinal member and in a second direction to release tension from the longitudinal member. A pivotal bone anchor assembly includes a shank with a head, an anchor portion, and a neck extending between the head and the anchor portion, and a receiver with upright arms defining a channel for receiving a rod and a central bore extending downward to a lower portion of the receiver. The assembly also includes a retaining structure rotatably coupled with the receiver with an internal seating structure pivotally supporting the head while providing for independent rotation of the shank prior to locking the assembly with the rod and a closure. The retaining structure includes an external surface and a slot extending between the seating structure and the external surface with a concave cut-out configured to closely receive the neck so as to provide for increased articulation between the shank and the receiver along the slot, and with the shank and the retaining structure being rotatable together around a centerline axis to align the increased articulation in any radial direction around the circumference of the receiver.

SURGICAL TETHER APPARATUS AND METHODS OF USE

A spinal treatment system includes a constraint device having an upper tether portion, a lower tether portion and a compliance member coupled therebetween. The upper tether portion is coupled with a superior spinous process of a spinal segment in a patient and the lower tether portion is coupled with an inferior spinous process or sacrum of the spinal segment. The length or tension in the constraint device is adjustable so that the construct of the tether portions and the compliance member provides a force resistant to flexion of the spinal segment. The system also includes a first prosthesis coupled with the spinal segment, wherein the constraint device modulates loads borne by the prosthesis or by tissue adjacent thereto.

METHODS AND SYSTEMS FOR LATERALLY STABILIZED CONSTRAINT OF SPINOUS PROCESSES

A spinal implant for limiting flexion of the spine includes a tether structure for encircling adjacent spinal processes. Usually, a pair of compliance members will be provided as part of the tether structure for elastically limiting flexion while permitting an extension. A cross-member is provided between the compliance member or other portions of the tether structure to stabilize the tether structure and prevent misalignment after implantation.

The shield can be coupled to the bone fixation assembly via separate coupling elements, such as a clip or an adjustable link secured between two vertebra joining members of the bone fixation assembly. Alternatively, the shield can include an integral attachment portion configured to engage the bone fixation assembly directly.
