A surgical system includes a trial connected with a first image guide oriented relative to a sensor to communicate a signal representative of the trial relative to a patient anatomy. A tracking device includes the sensor and communicates with a processor to generate a storable image of the trial relative to the patient anatomy for display from a monitor. A spinal implant is connected with a second image guide oriented relative to the sensor to communicate a signal representative of the spinal implant relative to the patient anatomy. The sensor receives the signal of the second image guide and communicates with the processor to generate an image of the spinal implant in real time for display from the monitor in a configuration to align the spinal implant in real time with the stored image of the trial. In some embodiments, methods, spinal constructs, implants and surgical instruments are disclosed.

A special stabilizing anchor is disclosed which is secured to the spinous process, in addition to anchors which are stabilized against the lateral masses. These anchors couple with the spinous process anchor and upon coupling, the connecting stabilizing element is configured such that this element can be actuated, elevating the spinolaminar arch and thus expanding the canal, relieving the stenosis and completing the surgical procedure. A unique aspect of this system is that the lateral mass anchors of different levels can be secured to each other, stabilizing one or more target motion segments. Augmenting this is a system for identifying and extirpating the facet joints and replacing them with graft material to encourage a posterior/facet fusion.

Multi-mode torque drivers employing inner surfaces compatible with pedicle screw guide wires, and related systems and methods are disclosed. A spinal column includes vertebrae in an articulating structure protecting a spinal cord. Medical intervention may involve limiting the relative motion between vertebrae by fusing vertebrae together with mechanical assemblies, including pedicle screws attached to the vertebrae. A multi-mode torque driver may be used to form the pedicle screw attachments with vertebrae. By including a passageway for a guide wire along an output rotational axis of a pedicle screw torque driver, pedicle screws may be inserted into a vertebra along a desired trajectory previously defined using the guide wire. In this manner, pedicle screws may be inserted into vertebrae precisely and efficiently.
