The invention is a solution for osteosynthesis made with molded metal sheets joined vertically and horizontally, fitted to the shape and dimensions of the fractured bone to keep it immobile and united. For the orthosis, it depicts metal sheets joined with horizontal and vertical movements, molded to the form and dimensions of vertebrae and joints to keep them separated. The sheet metals have wavy shapes, channels and undulations that give consistency and structural strength to bending, tension, shear and bends for joints. It also describes the process for the osteosynthesis with metal sheets and 3D printing, where the surgeon wraps the osteosynthesis or molded orthosis around the bone, vertebrae or joint, fastening it with clamps that tighten and lock with a surgical instrument, without screws. The sheet metals have viewpoint holes to see through during surgery and subsequent control with Roentgen. The clamps are in channels, which have micro-points on the side contacting the bone.

A fixation assembly for an implant comprises a screw having a head and a shaft, and a washer having at least one outer surface with one or more frictional elements for engaging a surface of the implant. The shaft includes one or more projections that cooperate with the washer to prevent the washer from sliding back down the screw, while allowing for easy application of the washer to ensure optimal placement and desired contact with the screw head. The washer and/or the screw head may also include interlocking cams that create a wedge effect that inhibits the screw from backing out or loosening from a hole in bone or other tissue.

One aspect of the disclosure relates to a surgical instruments and surgical instrument guides, and associated methods. The surgical instrument may include: an elongated body having a distal end; a housing disposed at least partially within the elongated body and pivotably coupled to the elongated body; and an actuator engaged with a proximal end of the elongated body and operably associated with the housing such that the housing pivots about a pivot point upon actuation of the actuator. In another embodiment, the surgical instrument may include: an elongated body having a segmented distal tip; a guide barrel extending longitudinally within the elongated body, the guide barrel being substantially aligned with a bore within the segmented distal tip, wherein the segmented distal tip is configured to transition between an unexpanded configuration and an expanded configuration.

The insertion instrument includes two arms connected at a hinge, a tip of each arm including a mating connector sized to engage a complementary mating connector on the wedge. In one embodiment of the method, the insertion instrument can be used to place the wedge plate in an osteotomy and where the wedge is a mobile wedge, a plate component can be adjusted to a desired position for securement while using the insertion instrument to hold the wedge in place in the osteotomy.
