An energy emitter is coupled to the elongate catheter and is configured to deliver ablative energy. A controller is received within the window and is coupled to the energy emitter such that axial movement of the controller within the window is translated to axial movement of the energy emitter and rotation of the controller within the window is translated into rotation of the energy emitter. The instrument includes a motor that is at least partially disposed within the housing of the catheter; a first gear that is operatively connected to and driven by the motor; and a second gear that is coupled to the energy emitter and is driven by the first gear to cause rotation of the energy emitter, while allowing the energy emitter to move axially.
METHODS, DEVICES, AND SUPPORT STRUCTURES FOR ASSEMBLING OPTICAL FIBERS IN CATHETER TIPS
A catheter includes proximal and distal sections, a shaft coupled between the proximal and distal sections, and optical fibers extending through the shaft and to the distal section of the catheter. The distal section includes a support structure that includes a proximal end, a distal end, reflective elements, and a cap disposed over a portion of the distal end of the support structure. The proximal end includes alignment receptacles. Each of the optical fibers is inserted into corresponding ones of the alignment receptacles and the alignment receptacles are shaped to maintain the optical fibers straight in the support structure. The distal end includes orifices facing different directions. Each of the optical fibers is optically aligned with corresponding ones of the lenses, reflective elements, and orifices such that the optical fibers in the support structure are straight. The cap includes optical ports aligned with the orifices.

The surgical laser system further comprises one or more beam splitters configured to reflect the one or more first diffracted beams and the one or more second diffracted beams onto a lens.
METHODS AND SYSTEMS FOR REDUCING NEURAL ACTIVITY IN AN ORGAN OF A SUBJECT
The present disclosure provides, according to some embodiments, methods and systems for selectively reducing, blocking or inhibiting at least part of the neural activity in an organ of a subject. In preferred embodiments, the method and system are used for selectively blocking at least part of the neural activity in a duodenum of a subject in need thereof. According to some embodiments, the selective blocking occurs through use of laser radiation. According to some embodiments, the selective blocking comprises causing damage to at least part of sensory nerves located within a target area while maintaining functional activity of tissue surrounding the sensory nerves. According to some embodiments, the sensory nerves include neurons configured to transmit signals triggered by food passing through the duodenum, such as, but not limited to, neurohormonal signals.
Ablation techniques for the treatment of atrial fibrillation
A catheter device provides a balloon structure and a side-firing laser lumen within the balloon to create lesions in the pulmonary vein (PV) in the treatment of atrial fibrillation. Mounted on the balloon so as to contact the PV when the balloon is inflated are one or more electrodes which may be used in a measurement mode, a treatment mode, or both.

DIFFUSING APPARATUS FOR LASER THERAPY TREATMENT
The present invention provides devices and methods of manufacture related to transmitting light to a target site for diffusion Techniques are provided which allow accurate control of the illumination profile with a diffuser tip design which is easily producible, relatively inexpensive, and provides variations to obtain desired illumination profiles. This is achieved by using at least two light scattering mediums having a shape defined by an insert. The dimensions, light scattering properties, and number of such light scattering mediums may be selected individually or collectively to selectively control the illumination profile. In addition, the insert allows for other beneficial design features, such as a reduced heat retention, easily controlled and refined light scattering medium interfaces, and a smaller cross-sectional diameter than is typically achievable with other techniques. The resulting light transmission and diffusion apparatus is operable with a high efficiency, highly predictable illumination profile, and ease of use.
