Suspensory graft fixation with adjustable loop length
A suspensory fixation device has an elongated anchor member adapted to be transversely situated at the exit of a bone tunnel. A graft supporting loop member formed of a pair of parallel suture limbs extending from a bight portion is suspended transversely from the anchor member and has a loop length which is adjustable so the graft ligament can be supported in the bone tunnel at varying distances from the anchor member. When a graft ligament is attached to the saddle end of the loop member, the length may be shortened by pulling distally on the pair of limbs to pull the graft ligament into the bone tunnel. When tension is applied to the loop member by the graft pulling the loop proximally, the bight portion of the suture automatically locks the graft supporting loop member in place.

A kit for trans-osseous rotator cuff repair comprising, a handle (101) with a hollow guide (106) and a first connector (104A), a hollow sleeve (103) that passes through the hollow guide (106) in the handle (101), a suture guider (105 and 503) to guide a surgical suture (516) through the hollow sleeve (103), an adaptable hook (107A and 107B) with a narrow slit (118) at one end and a second connector (104B) at opposite end to couple with the first connector (104A), and a pin (901) with first and second apertures (906 and 908) enabling knotless suture of a soft tendon tissue to a bone, wherein the hollow sleeve is adjustable within the hollow guide (106) and a tunnel is created within the bone by creating perpendicularly intersecting a first and a second hole (718 and 706) in that the narrow slit (118) of the adaptable hook (107A and 107B) retrieves the surgical suture (516) from the second hole (706) through the first hole (718) of the tunnel.
SUTURE ANCHOR
A knotless suture anchor system is disclosed for improved anchoring in tissue. The knotless suture anchor system includes an anchor body having an exterior surface, a proximal end, a distal end, a longitudinal axis extending between the proximal and distal ends, an interior longitudinal passageway extending at least partway from the proximal end toward the distal end, a proximal opening communicating with the longitudinal passageway nearer the proximal end, and a distal opening communicating with the longitudinal passageway nearer the distal end. The knotless suture anchor system also includes an interference member insertable distally into the longitudinal passageway to secure a portion of a suture within the longitudinal passageway by compressing the portion of the suture between the interference member and the anchor body, and a frangible connection that joins a proximal member to the anchor body.
Methods and systems for double loop stitching
In a method for providing a double loop stitch, a needle assembly having a first needle portion and a second needle portion that is removably connected to the first needle portion is provided. Opposing ends of a thread are connected to the needle portions, forming a loop that is placed around a sewing material. The needle assembly is inserted through the sewing material at an insertion point. The thread is then partially pulled through the sewing material at the insertion point such that a pair of loops of thread, separated by the insertion point, remain adjacent a first face of the sewing material. The needle portions are then separated and are passed around opposite sides of the sewing material from adjacent a second face back adjacent the first face. Each needle portion is then passed through one of the loops. The loops are then cinched and locked in place.

A graft fixation device comprising: a graft separator comprising a distal end, a proximal end, a cavity disposed between the distal end and the proximal end, and at least one guide rib disposed radially outboard of the cavity and extending between the distal end and the proximal end; and an interference screw rotatably mountable within the cavity, the interference screw comprising a distal end, a proximal end, and a screw thread disposed intermediate thereof, the screw thread disposed radially outboard of at least a portion of the graft separator and radially inboard of the at least one rib.
Ligament fixation system, implants, and devices with a compression cap, and methods of use
Implants, devices, systems, and methods for achieving ligament fixation are disclosed. An implant comprises a cap member comprising an internally threaded opening extending from a first end thereof, an anchor portion comprising a first end that defines a tip of the implant and external threads, and a coupling portion extending between the cap member and the anchor portion. The coupling portion of the implant includes an externally threaded portion threadably coupled within the internally threaded opening of the cap member. The cap member of the implant is longitudinally moveable along the coupling portion. Insertion instruments for inserting an implant for ligament fixation are also disclosed. Methods of using an implant for achieving ligament fixation are also disclosed.
APPARATUS AND METHOD FOR IMPLANTING BONE ANCHOR UNDER CONTROLLED TENSION
A graft tensioning tool includes an elongate frame having a proximal end, a distal end, and an axial passage extending therethrough. The distal end is configured to detachably engage a tunnel ring at an entrance to a tunnel in bone, and the frame includes a first suture puller and a second suture puller. Each suture puller detachably engages a length of suture emerging through the attached to suture and/or a soft tissue implant in the bone tunnel. A tensioner on the elongate frame displaces the suture pullers in a proximal direction to apply a controlled tension to the suture lengths and/or implant. A tension gauge measures tension applied to the suture pullers, and a locking tool is configured to engage the suture lock through the axial passage of the frame, allowing a user to fix the suture ends to bone when a target tension is attained.
Method and apparatus for coupling anatomical features
A method of coupling a first portion of an anatomy to a second portion of the anatomy includes coupling first and second bones of the anatomy with a bone-tendon-bone implant. The bone-tendon-bone implant has a first bone end, a second bone end and a replacement tendon between the first and second bone ends. The first and second bone ends are positioned in corresponding first and second bores of the first and second bones. The first bone end is secured to the first bone with a first bone anchor. A second anchor coupled to the bone-tendon-bone implant is passed through and outside the second bore. The second anchor is coupled to an adjustable suture construct having two adjustable suture loops. Tensioning at least one of first and second ends of the adjustable suture construct tensions the replacement tendon and positions the first bone relative to the second bone.

The invention provides surgical methods, devices, apparatus, and systems for supporting the thumb of a patient after basal joint arthroplasty. The embodiments of the invention comprise at least one index metacarpal anchor, at least one thumb metacarpal anchor, at least one sling for positioning the thumb metacarpal anchor relative to the index metacarpal anchor, and at least one anchor washer for tightening and securing the sling(s) without the need to tie the suture(s) components of the sling(s) into knots. Index and thumb metacarpal anchors comprise at least one bone-engaging element for engaging the respective metacarpal. The anchor washer may incorporate one or more sling-engagement mechanisms. Increasing the number of sling-engagement mechanisms associated with the anchor washer increases the surface area of the sling(s) and provides multiple sling elements to provide redundancy and increase the pull force of the sling(s) attachment(s) used to affix the thumb and index metacarpal anchors into a position relative to each other.

This disclosure provides an apparatus, system and method for ligament augmentation strap assembly. The ligament augmentation strap assembly includes an ligament augmentation strap inserted through a hole previously drilled through two adjacent bones; and a safe-release bone fastener attached to end of the ligament augmentation strap and configured to collapse when a force greater than a threshold is applied from the ligament augmentation strap.
