Apparatus is described for administering a substance to a subject. A vial contains the substance and a stopper is disposed within the vial and is slidably coupled to the vial. A first threaded element is (a) rotatable with respect to the vial and (b) substantially immobile proximally with respect to the vial during rotation of the first threaded element. A second threaded element is threadedly coupled to the first threaded element. At least a distal end of the second threaded element is substantially non-rotatable with respect to the vial, and the distal end of the second threaded element defines a coupling portion that couples the second threaded element to the stopper. The first threaded element, by rotating, linearly advances the stopper and at least the distal end of the second threaded element toward a distal end of the vial. Other embodiments are also described.

APPARATUS AND KITS FOR FLUID INFUSION

An infusion device includes a housing with an interior chamber sized and configured to hold at least a flange and plunger of a syringe, a trigger held by the housing, and a lever in communication with the trigger and including an upwardly extending cam with a cam path having an upper end. The cam is in communication with the flange of the syringe. In response to actuation of the trigger to dispense fluid from the syringe, the upper end of the cam travels upward above the syringe and longitudinally toward a dispensing end of the syringe to linearly translate the plunger of the syringe in a first direction to dispense fluid from the syringe. To refill fluid into the syringe, the upper end of the cam travels downward and longitudinally away from the dispensing end of the syringe to linearly translate the plunger in a second direction to intake fluid.

A metering device for dispensing medication fluid includes: a spindle unit, a drive unit for rotationally driving the spindle unit, and a reservoir for the medication fluid. The reservoir has a wall, which defines a cross-sectional area of the reservoir, and a plunger located in the reservoir. The rotational driving of the spindle unit causes a translational motion of the plunger, and therefore displaces medication fluid. Because the product of the cross-sectional area of the reservoir in the unit mm.sup.2 and the spindle pitch in the unit mm/degree is less than 0.13 mm.sup.3/degree and the medication fluid is a liquid insulin having a concentration in a range of U20 to U100, a metering system can be formed that is suitable specifically for the CSII therapy of children and youth, where high discharge accuracy is important for good therapy control.
