A spinal fixation device for securing two stabilizing rods to bone, such as the spine, pelvis or cranium, has a threaded shaft configured for fastening to bone, a first (primary) rod coupling head and a secondary rod coupling head. The threaded shaft has a longitudinal axis and the first rod coupling head is connected to the threaded shaft. The first rod coupling head has a side wall and a cavity for receiving a rod, the cavity being open toward a top of the first rod coupling head. In a neutral position of the first rod coupling head, the first rod coupling head extends symmetrically around the longitudinal axis of the threaded shaft. The second rod coupling head is rotatably connected to the first rod coupling head, around an axis of rotation. The axis of rotation is disposed at an angle that is not perpendicular to the longitudinal axis of the threaded shaft when the first rod coupling head is in the neutral position. This allows for two rods to be manipulated independently through different axis planes at any single point of bony fixation.

The rods are configured to be seated within receiver heads of the polyaxial bone screw assemblies when the screws are implanted in opposed lateral sides of a vertebra, thereby allowing the receiver heads to be manipulated relative to the rods to reduce one or more posterior fractures. A joint on the articulating connector can also be manipulated to reduce one or more anterior fractures. The entire construct can be locked to maintain the fractures in the reduced configuration.

A retention pin fixed to the yoke movably supports the saddle and a tension member within the bore. The tension member comprises a resilient biasing element oriented such that upward axial movement of the saddle produces a downward bias force on said saddle to compress the head of the bone fastener between said saddle and said socket collar in a provisional holding position.

A modular apparatus for extending a rod in an existing spinal construct comprises a modular rod connector for attachment to the existing rod and a modular rod extender that may be selectively attached to the rod connector. Each rod connector includes a spherical head that is formed to have the same size and configuration such that any selected modular rod extender may be attached to a selected rod connector. Each rod connector comprises an attachment portion having an opening for receipt of a portion of the rod of an existing spinal construct. Each modular rod extender comprises an extender body including a common modular attachment feature at one end for receipt of the spherical head of the selected rod connector, and an extension rod extending from an opposite end to extend the existing spinal rod.
