The invention involves a device and method for acquiring bone marrow aspirate. The system includes an elongated cannulated fastener having a threaded end for attachment to a bone. The threaded portion includes a directional aperture sized to allow marrow to be drawn into the cannula. The directional aperture also allows the user to direct the aperture in a desired direction within the bone for acquisition of the marrow. The distal end of the cannulated fastener includes a connector which may be in the form of a Luer lock to allow vacuum to be applied to the cannula for drawing the marrow into a syringe or the like.

The system provides for pivotal motion of the bone anchors in any of a plurality of angular directions relative to the vertical centerline axes of the multi-planar receivers while limiting pivotal motion to a single pivot plane relative to the vertical centerline axes of the uni-planar receivers.

A sacroiliac joint screw has a screw body having a head and a treaded shank with a self-drilling cutting tip. The screw body is cannulated and configured to receive a Steinmann pin for the purpose of a minimally invasive approach (MIS) for delivery to the sacroiliac joint (SI) while minimizing soft tissue damage. The self-drilling cutting tip creates a pilot hole. The threaded shank has a plurality of spiral cutting flutes, the spiral cutting flutes extend along a length of the threaded shank and are configured to provide a constant self-tapping feature. A plurality of bone harvesting windows are positioned in the spiral cutting flutes and are configured to pull in bone as the screw is advanced into the joint.

The distal end of the cannulated body has a tapered tip and a plurality of threads extend around the exterior of the cannulated body. An introducer extends distally from the tapered tip and has a diameter (which can be variable or constant) which is less than a diameter of the tapered tip. The surgical screw is cannulated such that a channel extends through the body from the proximal end to (and sometimes through) the distal end.

A bone fixation system includes a length of suture, a screw, a tensioning tab, and an anchor. The length of suture has a first end and a second end. The screw comprises an elongated shaft having a threaded external surface, wherein the screw has a distal end, a proximal end, and an internal surface defining a passageway within the screw extending between the distal end and the proximal end, wherein the passageway is configured to receive the length of suture. The tensioning tab is affixed to a section of the length of suture. The anchor is configured to secure the first end of the length of suture adjacent to the proximal end of the screw.

Some implants include a threaded distal region, an optionally threaded central region, and an optionally threaded proximal region. The distal, central, and proximal regions have lengths such that when the implant is laterally implanted across a SI joint, the distal region can be positioned in a sacrum, the central region can be positioned across an SI-joint, and the proximal region can be positioned in an ilium.
