An implant, prosthesis and method of use are described. The implant comprises an articulation component having a bearing surface and a reverse surface arranged to couple to a proximal neck portion of an arthroplasty implant coupled to a bone. A collar is arranged to couple to the reverse surface of the articulation component. The collar defines a neck hole arranged to pass around the coupling between the articulation component and the proximal neck portion of the arthroplasty implant. The collar and the articulation component comprise complementary coupling features to secure the collar to the articulation component. The collar further comprises a plurality of attachment portions to couple the collar to bone fragments or soft tissues. The prosthesis comprises the implant and an arthroplasty implant having a distal portion arranged to be coupled to an end of a long bone and a proximal neck portion.

An improved modular rotational device includes a first and second threaded coupler for affixation along the stem of an endoprosthetic device, for example, a humeral prosthesis or a femoral prosthesis. The rotational device axis of rotation is coaxial with the stem, and its axis of rotation is located in close proximity to the intramedullary stem of the prosthesis or in close proximity to the distal articulation of the prosthesis. A housing has a proximal and distal end with an axial bore therethrough for receiving an elongated stem of the device. A lobe ring may be utilized to limit the axis of rotation of the device. Additional endoprosthetic devices may be attached to male or female threaded couplers, or to Morse tapers. A plurality of suture attachments facilitates attachment of soft tissue thereto.

Methods and apparatus for treating disc herniation provide a conformable device which assumes a first shape associated with insertion and a second shape or expanded shape to occlude the defect which typically follows partial discectomy. The device may take different forms according to the invention, including patches size to cover the defect or plugs adapted to fill the defect. In a preferred embodiment, however, the device is a gel or other liquid or semi-liquid which solidifies to occlude the defect from within the body of the disc itself. In another preferred embodiment, a mesh screen is collapsed into an elongated form for the purposes of insertion, thereby minimizing the size of the requisite incision while avoiding delicate surrounding nerves. Such a configuration also permits the use of instrumentation to install the device, including, for example, a hollow tube or sheath adapted to hold the collapsed screen, and a push rod to expel the collapsed device out of the sheath for use in occluding the disc defect. A device according to the invention may further include one or more anchors to assist in permanently affixing the device with respect to the defect.

An expandable intervertebral device and its fixation during spinal surgery features the following: The intervertebral device is stabilized using a latticework of high-strength suture that is either directly integrated or external to the device and has hollow channels for threading of high-strength suture, permitting direct integration of suture into the device for fixation via bone anchors. Expansion and retraction of the intervertebral device is achieved by modulating a dual-access screw, which can be accessed from either end of the intervertebral device. The versatility of the dual-access screw and easily detachable suture threads afford surgeons an ability to remove the intervertebral device from the most optimal direction during primary or revision surgery. This intervertebral device having the dual-access screw and high-strength suture fixation permits a secure, safe, versatile, and efficient approach to spinal surgery.

APPARATUS FOR THE TREATMENT OF TISSUE

A system for treatment of tissue includes a fixation apparatus and a delivery tool releasably carrying the fixation apparatus. The fixation apparatus includes first and second anchors and an adjustable band connecting the anchors. The band includes a cinch line and a tether connected to one of the anchors. The delivery apparatus includes a body, a tubular shaft, a displacement rod within the tubular shaft, and an actuator.

ROTATABLE COLLAR FOR A PROSTHESIS

A collar arranged to couple to a prosthetic component for implantation at the end of a long bone, the prosthetic component having a distal portion arranged to couple to the bone and a proximal neck portion arranged to couple to an articulation component. The collar comprises a plate including a neck hole to receive a neck portion, such that the collar can rotate about the neck portion. The collar further comprises a plurality of attachment portions for coupling the plate to bone fragments or soft tissues. The collar is lockable to the neck portion to prevent further rotation. A prosthesis comprising a combination of a collar, a prosthetic component and an articulation component, and a surgical method of using the collar are also disclosed.
