A spinal interbody cage implant includes a cage, an endplate, and flexible barbs. The endplate is separately attached to the cage and is configured to receive, hold and direct a flexible barb into an upper vertebral surface and a flexible barb into a lower adjacent vertebral surface. Each proximal leg of the cage is configured to receive the endplate, and has a lateral bore that receives a pin which retains the endplate. The endplate directs a first flexible barb upwardly toward an upper vertebral surface, and directs a second flexible barb downwardly toward a lower vertebral surface. Each flexible barb has a proximal head, a shaft extending from the head with a bore extending from the head to its distal end. Teeth are provided along the exterior surface of the shaft with two flats disposed on opposite sides thereof with a slit extending from the distal end towards the head.

An implant assembly may include a first component having a proximal plate and a distal portion extending from the proximal plate, a first screw of a first type, and a first screw of a second type. An opening may be defined by the proximal plate and extend through the distal portion. Each of the plurality of apertures may be disposed between the opening and the periphery. The first screw of the first type may be sized and configured to be received in the opening and engage bone. The first screw of the second type may be sized and configured to be received in at least one of the plurality of apertures.

Adjacent surfaces of at least three adjacent layers have a substantially identical pore pattern aligning to interconnect the pores of the at least three adjacent layers to form a continuous porosity through the at least three adjacent said layers.

The arm is disposed through bone in the space between successive portions of the helical structure when the prosthesis assembly is implanted.
