This application describes surgical instruments and implants for building a posterior fixation construct across one or more segments of the spinal column. Extension guides are provided that attach to bone anchors implanted within the spine. The extension guides have a guide channel that align with a rod channel in the anchor to help direct the rod to the anchor. Instruments are provided to aid in insertion and positioning or the rod.

A fastener system couples the elongated fixation rod to the skeletal structure. The fastener system includes an anchor that is to be secured to the skeletal structure, a saddle coupled to the anchor, and a locking member that maintains cooperation between the saddle and the portion of at least one of the different rod regions interfere with separation of the fixation rod from the fastener system.

A pedicle screw-rod system capable of gradually changing from rigid fixation to non-rigid fixation is provided. The system includes a pedicle screw, a connecting rod, and a screw plug. The connecting rod is used for connecting in series the pedicle screw fixed to adjacent vertebrae on the same side of a spine, and the screw plug is used for locking the connecting rod on the pedicle screw intersecting therewith. At least one of two structural members of the pedicle screw or the connecting rod is connected by a flexible connecting part, and the flexible connecting part is also connected to degradable stoppers for preventing flexible parts from swinging with each other; and with gradual degradation of the degradable stoppers, a fixation manner of the pedicle screw-rod system will be gradually converted from initial rigid fixation into non-rigid fixation.

The system further includes an adjustable portion configured for affixation relative to the subject's spine at a second location remote from the first location, the adjustable portion comprising a housing containing a magnetic assembly, the magnetic assembly affixed at one end thereof to a lead screw, the lead screw operatively coupled to the threaded portion. A locking pin may secure the lead screw to the magnetic assembly. An O-ring gland disposed on the end of the housing may form a dynamic seal with the distraction rod.

Various devices and techniques are described for transition from a rigid fixation construct to a less rigid support structure applied to a “soft zone” that helps share the stress created on the spinal levels caused by the fixed levels below. In some embodiments, the soft zone is provided by terminating the construct with one of a flexible tether or a dampening rod.

Minimally Invasive Surgical System

A multi-stage minimally invasive surgical procedure and associated instruments are disclosed. First, the surgical site is prepared. After preparation, the bone screws or anchors are attached to the bone. Subsequent to insertion of the screws, a rod or connecting member is positioned within the yoke portion of the bone screw. Caps are then placed in a pre-lock position within the yokes. The bone screws may be compressed together or distracted along the rod or connecting member, thereby setting the final spacing of the bones or bone segments. Finally the caps are moved to a final lock position to fix the screws to the rod or connecting member to maintain the bones in position relative to each other.

Correction of a scoliotic curve in a spine includes the steps of implanting an expanding rod isolated completely under the skin and attached to selected portions of the scoliotic curve of the spine at opposing ends of the rod; and producing a controlled force by means of expansion of the rod over at an extended time period under external control until a desire spinal curve is obtained. An incremental force is generated to stretch the scoliotic curve of the spine between the selected portions where attachment of the rod is defined. The controlled force is provided steadily for at least one month or alternatively 1-3 months. Multiple rods may be employed with each associated with a different scoliotic curve of the spine or a different portion of the scoliotic curve.

The insertion tool further includes a threaded portion for cooperation with the reduction tool to provide synchronized placement of a closure structure in the bone screw receiver while reducing and capturing a longitudinal connecting member within the receiver. Further alternative bone screws are hinged, polyaxial or fixed and include lordosing or kyphosing lateral surfaces.
