A pivotal bone anchor assembly includes a receiver having upright arms defining a receiver channel for receiving an elongate rod, and an axial bore extending downward from the receiver channel to a bottom opening. The upright arms include break-off extensions, outer horizontally extending curvate tool engagement grooves, and side apertures extending inwardly from outer surfaces of the upright arms to deformable crimp wall portions adjacent the axial bore. The assembly also includes a shank having an anchor portion and an integral upper portion that is positionable in the axial bore of the receiver with the shank extending downward through the bottom opening. The assembly further includes an insert with an insert channel positionable within the axial bore and having opposing depressions formed into side surfaces that are configured to cooperate with the crimp wall portions of the receiver to maintain the insert channel in alignment with the receiver channel.

Furthermore, the system includes an outer shaft extending longitudinally and including a channel extending therethrough. The channel of the outer shaft is sized and shaped to slidably receive the main body therein and including a retaining feature extending along a distal end thereof. The retaining feature of the outer shaft engages a retaining structure extending along an exterior surface of the second screw.

Method of using bone plate with polyaxial injection screws

A method is provided for stabilizing a bone. A plate is positioned over a bone. The bone plate has a polyaxial screw hole with a spherically curved seat. A bone hole is formed beneath the screw hole. A screw having a head with a spherical outer surface, and a threaded shaft with an internal cannula and a plurality of fenestrations is provided. The screw is driven through the first screw hole and into the bone hole until the outer surface of the head is supported by the spherically curved screw seat. A syringe provided with a needle is inserted into the internal cannula of the shaft of the screw. The syringe includes a bone injectable material that is injected through the needle, into the internal cannula, and out of at least one fenestration into the bone. After injection, the needle is removed from the screw shaft.
