    Abstract

    A catheter device includes a tube, and an extracorporeal unit coupled to a proximal end of the tube. An implant includes a series of anchors and a tether. The series of anchors includes a first anchor and other anchors, arranged along the extracorporeal unit. Each of the anchors includes a tissue-engaging element, and a head that defines an eyelet. The tether extends, along the extracorporeal unit, through the eyelet of each of the other anchors to the first anchor such that (i) the first anchor is advanceable, with a distal end of the tether, distally into and through the tube toward the tissue, and (ii) subsequently, the other anchors are slidable, sequentially, over and along the tether, distally into and through the tube toward the first anchor. Other applications are also described.

    Claims

    1. A system for use with a tissue of a subject, the system comprising: a catheter device, comprising: a tube that has: a distal opening that is configured to be transluminally advanced to the tissue, and a proximal end that defines a proximal opening, and an extracorporeal unit that is coupled to the proximal end of the tube; and an implant, comprising: a series of anchors, including a first anchor and other anchors, the series of anchors being arranged along the extracorporeal unit, each of the anchors comprising: a tissue-engaging element, and a head that defines an eyelet, and a tether that extends, along the extracorporeal unit, through the eyelet of each of the other anchors to the first anchor such that: the first anchor is advanceable, with a distal end of the tether, distally into the proximal opening and through the tube toward the tissue, and subsequently, the other anchors are slidable, sequentially, over and along the tether, distally into the proximal opening and through the tube toward the first anchor.

    2. The system according to claim 1, wherein the catheter device comprises a series of cartridges, arranged along the extracorporeal unit, each of the cartridges holding a corresponding anchor of the series of anchors.

    3. The system according to claim 2, wherein each of the cartridges: has an initial position in which the cartridge inhibits sliding of the corresponding anchor with respect to the extracorporeal unit, and while remaining coupled to the extracorporeal unit, is actuatable in a manner that releases the corresponding anchor from the cartridge.

    4. The system according to claim 3, wherein each of the cartridges is configured such that actuation of the cartridge spaces the corresponding anchor from a barrier that, in the initial position of the cartridge, obstructs the corresponding anchor from being advanced distally out of the cartridge.

    5. The system according to claim 3, wherein, for each of the cartridges, while the cartridge is in its initial position, the cartridge inhibits sliding of the corresponding anchor distally along the tether and through the proximal opening into the tube.

    6. The system according to claim 5, wherein, for each of the cartridges, the cartridge is configured such that actuation of the cartridge releases the corresponding anchor to be advanced distally along the tether and through the proximal opening into the tube while the cartridge remains coupled to the extracorporeal unit.

    7. The system according to claim 3, wherein, for each of the cartridges, the cartridge: comprises a piece that cradles the corresponding anchor, and is actuatable by moving the piece proximally.

    8. The system according to claim 7, wherein the piece couples the cartridge to the extracorporeal unit.

    9. The system according to claim 3, wherein, for each of the cartridges, the cartridge is actuatable by pulling the corresponding anchor proximally.

    10. The system according to claim 3, wherein each of the cartridges is configured such that, subsequently to the cartridge being actuated, the corresponding anchor is advanceable distally out of the cartridge while the cartridge remains coupled to the extracorporeal unit.

    11. The system according to claim 1, wherein the extracorporeal unit comprises a tensioner that comprises a spring, and that is configured to reduce slack on the tether.

    12. The system according to claim 11, wherein the tensioner is configured to reduce the slack by winding-in the tether.

    13. The system according to claim 12, wherein the tensioner comprises a winch that comprises: the spring, and a spool, coupled to the spring such that rotation of the spool in a first direction applies stress to the spring.

    14. The system according to claim 13, wherein a proximal portion of the tether is wound around the spool such that advancement of the first anchor, with the distal end of the tether, distally into the proximal opening and through the tube, rotates the spool in the first direction.

    15. The system according to claim 11, wherein the tensioner comprises a clamp, and is configured to reduce the slack by the spring proximally sliding the clamp while the clamp is clamped to the tether.

    16. The system according to claim 1, wherein the system is configured such that the first anchor is advanceable, with the distal end of the tether, distally into the proximal opening and through the tube toward the tissue while the other anchors remain arranged along the extracorporeal unit.

    17. The system according to claim 1, wherein the system is configured such that the first anchor is advanceable, with the distal end of the tether, distally into the proximal opening and through the tube toward the tissue while, for each of the other anchors, the tether remains threaded through the eyelet.

    18. The system according to claim 1, wherein the system is configured such that the first anchor is advanceable, with the distal end of the tether, distally into the proximal opening and through the tube toward the tissue, while a proximal end of the tether remains coupled to the extracorporeal unit.

    19. The system according to claim 1, further comprising an anchor driver, configured to, for each of the anchors sequentially, engage the head and advance the anchor distally into the proximal opening and through the tube.

    20. The system according to claim 1, wherein: the catheter device further comprises a port at the proximal opening of the tube, the port comprising a membrane, and the membrane is shaped to define a first aperture through the membrane, a second aperture through the membrane, and a closed slit connecting the first aperture with the second aperture.

    21. A system for use with a subject, comprising: an implant, comprising: a tether, and a series of anchors, arranged along the tether, each of the anchors comprising: a tissue-engaging element, and a head that defines an eyelet, the eyelet being threaded onto the tether; and a catheter device, comprising: a tube that has: a distal opening that is configured to be advanced into the subject, and a proximal end that defines a proximal opening, and an extracorporeal unit that is coupled to the proximal end of the tube; and a series of cartridges, arranged along the extracorporeal unit, each of the cartridges: holding a corresponding anchor of the series of anchors, being coupled to the extracorporeal unit in a respective initial position in which the cartridge inhibits sliding of the corresponding anchor distally along the tether and through the proximal opening into the tube, and while remaining coupled to the extracorporeal unit, being actuatable in a manner that releases the corresponding anchor from the cartridge.

    22. An anchor driver for use with a tissue anchor shaped so as to define a driver interface having a proximal rim and a recessed portion surrounded by a wall of the tissue anchor extending distally from the proximal rim, the anchor driver comprising: a flexible tube; a driver head coupled to the flexible tube, the driver head being shaped so as to a deflectable protrusion having a resting state in which the protrusion assumes a collapsed state toward a central axis defined by the flexible tube and does not contact the tissue anchor; a shaft disposed within a lumen of the flexible tube and engageable with the deflectable protrusion such that when the shaft slides alongside the deflectable protrusion, the shaft deflects the deflectable protrusion from the collapsed state, away from the central axis, and into an engaged state of the deflectable protrusion in which the deflectable protrusion engages with the tissue anchor; and first and second expander elements coupled to an outer surface of the flexible tube and expandable proximally to the tissue anchor, the first and second expanded elements having respective distal surfaces at each end of the first and second expander elements which, in the engaged state of the deflectable protrusion, push against the proximal rim of the tissue anchor to facilitate pushing of the tissue anchor.

    DETAILED DESCRIPTION

    [0282] Reference is made to FIGS. 1A-B and 2A-B, which are schematic illustrations of examples of a catheter system 40 comprising a transluminally-advanceable catheter 12, at least one tissue anchor 220, and an implant 210 comprising the tissue anchor, and techniques for use therewith, in accordance with some applications. System 40 is a tissue-adjustment system can be used for adjusting a dimension of a tissue structure. For example, system 40 can be an annuloplasty system, and implant 210 can be an annuloplasty structure.

    [0283] System 40 comprises an extracorporeal unit (e.g., a controller and/or handle) 18 at a proximal portion of system 40. Transluminally-advanceable catheter 12 is coupled to extracorporeal unit 18 at a proximal end of catheter 12 and extends distally therefrom along a longitudinal catheter axis 16 of catheter 12, from unit 18 to a distal opening 14 of catheter 12. Transluminally-advanceable catheter 12 comprises a channel 254, e.g., a flexible tube, having a generally tubular wall that defines a lumen along catheter axis 16. For applications in which tissue 10 represents tissue of the annulus of a native heart valve, such as the mitral valve, implant 210 is an annuloplasty structure comprising a wire 212 and multiple anchors 220. Driver interface 282 is configured to be reversibly engaged by a flexible anchor driver 260. Driver 260 can comprise an elongate and flexible shaft 261, and a driver head 264 coupled to a distal end of the shaft. This is hypothesized to be useful, inter alia, for applications in which the wire is tensioned after implantation in order to adjust anatomical dimensions, such as annuloplasty.

    [0289] Transluminally-advanceable catheter 12 comprises a helical thread 20 overlapping with, e.g., coaxial with, channel 254 and operably connected to extracorporeal unit 18 such that, via operation of unit 18, thread 20 is rotatable about catheter axis 16 with respect to channel 254. Transluminally-advanceable catheter 12 has an anchor-storage zone 22 in which thread 20 is longitudinally at least partly coincident with slit 256 of channel 254. The coincidence of thread 20 and channel 254 defines anchor-storage zone 22. In some applications, thread 20 and channel 254 are concentrically disposed. The plurality of anchors 220 of implant 210 are disposed within anchor-storage zone 22. For some applications, transluminally-advanceable catheter 12 comprises an outer tube 252. For applications in which catheter 12 comprises outer tube 252, outer tube 252 is shaped so as to define a distal linear slit which extends from opening 14 of catheter 12 proximally and aligns with slit 256 of channel 254. For such applications, catheter 12 defines an inner tube disposed between outer tube 252 and channel 254, and thread 20 is coupled to a distal end of the inner tube in a manner in which rotation of the inner tube rotates thread 20. Often, as shown, thread 20 defines a tubular structure. For some applications, thread 20 is created from the inner tube by cutting away a helical slit in the inner tube. For either application in which thread 20 is coupled to a distal end of the inner tube or is formed from the inner tube, thread 20 is rotatable with respect to both channel 254 and outer tube 252. For some applications, catheter 12 does not comprise an inner tube, rather the inner surface of outer tube 252 defines a helical groove which defines thread 20 in a manner in which rotation of outer tube 252 facilitates rotation of thread 20 with respect to channel 254. For some applications, outer tube 252 comprises a polymer.

    [0290] Anchor-storage zone 22 has a distal end that is disposed proximally to distal opening 14 of catheter 12. In such a manner, the distal portion of catheter 12 is devoid of anchors 220 to enable flexibility and manipulation of the end portion of catheter 12. The distal end of anchor-storage zone 22 can be at a distance of no more than 90 cm from distal opening 14, such as no more than 25 cm, to maintain the comfort and stability level of the user. For some applications, the distal end of zone 22 is at a distance of between 3 cm and 25 cm from distal opening 14, such as between 5 cm and 25 cm.

    [0294] Thread 20 is shaped so as to define a plurality of adjacent ribbons 24, or coils, arranged helically. The plurality of ribbons have a pitch 28 between adjacent ribbons. Each ribbon 24 has a surface 26 defining a depth of the ribbon 24. Protrusion 241 of each anchor 220 rests against a respective surface 26 of a given ribbon 24 and in pitch 28 between adjacent ribbons. As shown in FIG. 1A, a first, distal-most tissue anchor 220a, prior to rotation of thread 20 and while thread 20 is stationary, is positioned at a first location 30a with respect to channel 254, distally to a second tissue anchor 220b that is disposed in a second location 30b proximal to first location 30a.

The spring is configured to facilitate distal movement of tissue anchors 220 and is arranged to apply a distally-directed force to the proximal-most anchor 220 within anchor-storage zone 22, thereby holding anchors 220 within storage zone 22, and helping to facilitate distal advancing of the remaining anchors 220 as thread 20 is rotated and each of anchors 220 is separately deployed.

    [0297] It is to be noted that rotation of thread 20 by less than a complete turn, e.g., 180 degrees, provides the operating physician with an indication of the connection between driver 260 and anchor 220. Additionally, the interaction between thread 20 and anchors 220 functions as a safety mechanism to prevent premature release of an anchor 220.

    [0298] FIGS. 2A-B show system 40, which comprises implant 210, and a delivery tool 250 for percutaneous (e.g., transluminal, such as transfemoral) implantation of the implant. Tool 250 comprises flexible anchor driver 260 that is configured to reversibly engage driver interface 282 of anchor 220. Via this engagement, driver 260 is configured to drive tissue-engaging element 230 into tissue, e.g., by rotating (and distally pushing) anchor 220. In some applications, tool 250 further comprises flexible tube 252 (e.g., a transluminal catheter) via which each anchor 220, engaged with driver 260, is advanceable to the tissue to which the anchor is to be anchored.

    [0299] In FIG. 2A, multiple anchors 220 have been anchored to tissue 10. Each anchor 220 was delivered to the tissue in a delivery state in which wire 212 extends through and is slidable with respect to aperture 246 of eyelet 240 while generally parallel to axis ax2. This is illustrated for an anchor 220d that is shown in FIG. 2A as being currently delivered. A straight path is possible for wire 212 to take through aperture 246 of eyelet 240.

    [0300] As subsequent anchors 220 are anchored to the same tissue, wire 212 becomes oriented laterally with respect to the anchors. Due to the configuration of eyelet 240, despite this reorientation of wire 212, the wire can still take a clear straight path through aperture 246 of eyelet 240.

    [0301] After a desired number of anchors 220 have been anchored, an adjustment tool is introduced (e.g., over and along a proximal portion of wire 212) and is used to facilitate tensioning of the wire. Therefore, the tensioning of wire 212 draws anchors 220 closer together, thereby contracting the tissue to which the anchors are anchored. This is facilitated by eyelets 240 providing smooth sliding of wire 212 through apertures 246 while the wire is orthogonal to the anchors, as described hereinabove. Respective stoppers 214 are coupled to wire 212 to prevent wire 212 from sliding out of anchors 220. FIG. 2A shows a stopper distal to first anchor 220a. Excess wire 212 can then be cut and removed from the patient.

    [0302] For simplicity, FIG. 2A shows implant 210 in a linear configuration. However, for annuloplasty, implant 210 is often implanted in a curve (or even a complete ring) around the valve annulus, such that the contraction reduces the size of the valve annulus, improving coaptation of the valve leaflets.

    [0303] As described hereinabove, for some applications eyelet 240 is mounted to be revolvable around axis ax2. This therefore provides independence between the rotational position of the eyelet and that of tissue-engaging element 230. It is hypothesized that, for applications in which tissue-engaging element 230 is helical, this independence advantageously allows the tissue-engaging element to be screwed into tissue to the extend needed for optimal anchoring, without a requirement for the anchor to finish in a particular rotational orientation. It is further hypothesized that, irrespective of the type of tissue-engaging element 230 used, this independence allows eyelet 240 (and wire 212) to be in an optimal position, with respect to axis ax2 of each anchor 220, for a given application. For example, for an application in which implant 210 is used for annuloplasty, anchors 220 are often anchored in a curve around the valve annulus, and eyelets 240 and wire 212 can be disposed on the inside of the curve relative to axis ax2.

    [0304] To anchor 220, the anchor is advanced out of a distal end of channel 254 and of tube 252 while driver 260 rotates driver interface 282 (and thereby tissue-engaging element 230) with respect to the tube, and while slit 256 can inhibit rotation of ring 284 with respect to the tube. For some applications, it is advantageous for the distal end of tube 252 to be disposed (or even pressed) against tissue 10 during anchoring of the anchor, e.g., as shown in FIG. 2A. For applications in which tube 252 is used to implant an implant comprising multiple anchors on a wire, such as implant 210, interference may occur between the wire and the contact between the distal end of the tube and the tissue. For some applications, tube 252 defines a lateral slit extending proximally from the distal end of tube 252 and channel 254 defines slit 256 extending proximally from the distal end of channel 254, such that the slits are continuous with distal opening 14 of catheter 12. For some applications, the slits are adjacent to (e.g., laterally outward from) the lumen of catheter 12, and allows wire 212, but not anchor 220, to exit tube 252 and channel 254 laterally, proximally from distal opening 14. It is believed that this facilitates implantation of implants such as implant 210, comprising multiple anchors coupled to (e.g., threaded on) a wire.

    [0305] Each anchor 220 is shaped so as to define a respective tissue-anchor-lumen extending through a longitudinal length of anchor 220 from a proximal end of anchor head 280 to a distal end of tissue-engaging element 230. Anchor driver 260, e.g., particularly driver head 264, is dimensioned to be axially slid within the lumen of channel 254 and is actuatable to reversibly engage the driver interface 282 of each tissue anchor within anchor-storage zone 22. Anchor driver 260 is configured to, while driver head 264 is engaged with driver interface 282, advance anchor 220 distally away from anchor-storage zone 22, toward distal opening 14 of catheter 12, and drive tissue-engaging element 230 along tissue anchor axis ax2 into tissue 10. Driver 260 advances tissue anchor 220 distally together with rotation of thread 20. In the absence of rotation of thread 20, anchor driver 260 is prevented from advancing anchor 220 distally.

    [0306] Each anchor 220 defines the respective tissue-anchor-lumen, and anchor driver 260, e.g., particularly driver head 264, is dimensioned to be axially slid within all of the tissue-anchor-lumens of anchors 220. For example, when the distal-most anchor 220 is engaged by driver 260, driver 260 passes through the tissue-anchor-lumens of all of anchors 220 until it reaches anchors head 280 of the distal-most anchor. Once driver 260 implants the distal-most anchor 220, it is disengaged from anchor 220 and retracted through the lumen of channel 254 and through the tissue-anchor-lumen of the next anchor 220 in sequence that was disposed proximally adjacent to the distal-most anchor.
