COMPOSITIONS AND SYSTEMS FOR RENAL FUNCTION DETERMINATION

The present disclosure relates to systems and methods for determining the renal glomerular filtration rate or assessing the renal function in a patient in need thereof. The system includes a computing device, a power supply, one or more sensors, and at least one tracer agent that fluoresces when exposed to electromagnetic radiation. The electromagnetic radiation is detected using the sensors, and the rate in which the fluorescence decreases in the patient is used to calculate the renal glomerular filtration rate in the patient.

APPARATUS FOR EFFECTIVE ABLATION AND NERVE SENSING ASSOCIATED WITH DENERVATION

An intravascular catheter for nerve activity ablation and/or sensing includes one or more needles advanced through supported guide tubes (needle guiding elements) which expand to contact the interior surface of the wall of the renal artery or other vessel of a human body allowing the needles to be advanced though the vessel wall into the extra-luminal tissue including the media, adventitia and periadvential space. The catheter also includes structures which provide radial and lateral support to the guide tubes so that the guide tubes open uniformly and maintain their position against the interior surface of the vessel wall as the sharpened needles are advanced to penetrate into the vessel wall. Electrodes near the distal ends of the needles allow sensing of nerve activity before and after attempted renal denervation. In a combination embodiment ablative energy or fluid is delivered from the needles in or near the adventitia to ablate nerves outside of the media while sparing nerves within the media.

At least one of the one or more sensors are configured to sense a substance of interest. The catheter insert elongated body includes a material that is a substantially non-permeable to the substance of interest.

A method for measuring a glomerular filtration rate in a mammalian kidney comprises a source of reporter and marker fluorescent molecules. The fluorescent molecules are introduced into the blood stream of a mammalian subject. Over a period of time, a measurement of the intensities of the reporter and marker fluorescent molecules is taken. A ratio is calculated to determine the health of the subject's kidney. This method measures volume of plasma distribution based on a fluorescence of a marker molecule relative to a fluorescence of a reporter molecule.

RENAL DYSFUNCTION RISK STRATIFICATION

Systems and methods for assessing a patient's risk of renal dysfunction are described. A system may include sensor circuits to sense physiological signals and processors to generate signal metrics from the physiological signals. The system may generate a primary renal risk indication using a first signal metric, and a secondary renal risk indication using at least a second signal metric. The system may generate a composite renal risk indication and estimate a glomerular filtration rate or a chronic kidney disease stage using at least the primary and secondary risk indications. The composite renal risk indication, which indicative of a degree of renal dysfunction, may be presented to a clinician, or provided to a detector for detecting worsening heart failure.
