The more accurate sizing and placement of the cross-pinned implants additionally allow both ends of the implants to recess beneath or be flush with the outer surface of the bone, which can reduce the risk of infection. The guide wire can allow the implants to be inserted using a cannulated technique for easier surgical procedure and better outcome, with the implants having sufficient wall thickness for the desired length to achieve bicortical purchase while keeping the shaft outer diameter of the implant to be similar to a diameter of the guide wire.

Using the handle to implant the implant, along with an associated spacer, provides a quick and convenient method for implanting the implant as described herein and in the associated drawings.

The cone extensions include a plurality of ridges that will catch bone on insertion and provide a press fit. The present invention also includes a cone and cup reaming system wherein one or more reamers will ream a cone/cup shape for the articular surfaces, and prepare the metatarsal and phalangeal canals for the insertion of the cone extensions. The cone extensions have ends that fit the reamed cone/cup shape wherein the cone/cup shape resembles the natural anatomy of articular surfaces of metatarsophalangeal joint.
