A device for engaging tissue includes a body disposable around the distal end of a medical instrument, such as an endoscope, and a handpiece. The body includes a primary channel for receiving the medical instrument and at least one arm channel for receiving an arm therethrough. At least one arm extends through the arm channel of the body and proximally to the handpiece. The arm includes an engagement member distally of the body, and may further include a bend between the body and engagement member. The engagement member is movable between a first position not contacting tissue and a second position contacting tissue. With multiple engagement members, the engagement members are farther apart from one another in the first position and closer together in the second position. Movement of the engagement members is controlled by selective and independent movement of the corresponding arms by rotational and/or translational motion.

A light transmitting medical instrument provides for the enhanced performance of medical procedures. The light transmitting medical instrument comprises a light pipe having a first end operatively coupled to a light source and a second end having a functional tip having at least two arms. A sliding closure sleeve is mounted about the light pipe. The sliding closure sleeve has a first end portion at which the sliding closure sleeve is fixed to the light pipe in a manner such that relative movement therebetween is prevented and a second end proximate the functional tip of the light pipe. The sliding closure sleeve further comprises a spring-like actuator finger grip that, when forced radially inward, causes the second end of the sliding closure sleeve to move toward the functional tip, thereby forcing the arms of the functional tip towards one another.

The apparatus further includes an operating device to operate the first and second holding devices, wherein the operating device includes a first operating organ biased in the normal position by one or more second spring elements with a second biasing force, wherein the first biasing force is smaller than the second biasing force such that exerting an actuation force on the first operating organ first results in movement of the first and second jaw element towards each other, and subsequently in actuation of the operating device.
