A surgical sheath for use in endoscopic trans-nasal or intra-ocular surgery is made of a braid material. The sheath may be manufactured by placing a length of braided tube material over a mandrel. The braid material is conformed to the shape of the mandrel and is then heat set. An atraumatic end may be made by folding or rolling one or both ends of the sheath. A coating may also optionally be applied to the braid material. The sheath reduces collateral trauma to the tissues in the surgical pathway.

A tympanostomy tube delivery device comprises a drive assembly and a force limiting shaft assembly. The shaft assembly comprises a cannula and a piercer operable to translate relative to the cannula so as to pierce the tympanic membrane of a patient. The piercer is mechanically coupled with the drive assembly such that actuation of the drive assembly causes translation of the piercer relative to the cannula. The piercer is mechanically coupled with the drive assembly by a clutch. The clutch is resiliently biased to couple the piercer with the drive assembly. The clutch is configured to uncouple the piercer from the drive assembly in response to a threshold force being imposed upon the piercer so as to inhibit translation of piercer relative to the cannula.

The distal end of the shaft may be shaped for tissue penetration. The instrument may further include a composite microcannula slidably engaged with the trocar in the lumen. The microcannula includes a light guide and a flexible hollow tube having an outer diameter less than an inner diameter of the lumen in the trocar. Other embodiments include placing the microcannula in the lumen of the trocar, illuminating the end of the trocar by illuminating the end of the microcannula, advancing the trocar from a selected entry point on an eye into a selected structure in the eye, and extending the illuminated end of the microcannula from the trocar into the selected structure.
