Provided is a scissors-type medical treatment tool capable of not only local tissue fixation in an incision/cutting part but also heating on side surfaces of electrodes of a forceps/tweezers type. It has been found out that a tissue part in which tissue is nipped between blades and a tissue part which is brought into abutment against side surfaces of the blades can be heated simultaneously or sequentially by optimizing arrangement of microwave application electrodes and microwave receiver electrodes in a blade pair (in particular, by causing coagulation to be performed evenly on the left and right of the blades), and the present invention has therefore been achieved.
ENERGY BASED SURGICAL INSTRUMENTS AND SYSTEMS
A surgical system includes a housing, an ultrasonic transducer supported by the housing, an end effector assembly distally-spaced from the housing, a clamp lever, and a sensor. The end effector assembly includes an ultrasonic blade operably coupled to the ultrasonic transducer for receiving ultrasonic energy produced by the ultrasonic transducer, and a jaw member pivotable relative to the blade from an open position towards a clamping position for clamping tissue between the jaw member and the blade and imparting a clamping force to the clamped tissue. The clamp lever is operably coupled to the housing and the jaw member such that actuation of the clamp lever relative to the housing pivots the jaw member towards the blade. The sensor is configured to sense whether the clamp lever is sufficiently actuated so as to impart the clamping force to the clamped tissue.

In an energy control device, a processor detects a gradual decrease start time at which the electric characteristic value in relation to electric energy output to an ultrasonic transducer starts a gradual decrease after a gradual increase. The processor calculates a difference value by subtracting the electric characteristic value from a peak value at gradual decrease start time and calculates an integrated value of the difference value from the gradual decrease start time. The processor executes, based on a fact that the integrated value become greater than a predetermined threshold, at least one of causing to stop or reduce the output of the electric energy to the ultrasonic transducer, and notifying that the integrated value become greater than the predetermined threshold.

An electrosurgical generator includes a processor and a memory storing instructions executable by the processor. The instructions when executed, cause the generator to provide an indicated treatment energy to the instrument, where the indicated treatment energy is set by a user and having a corresponding current limit, receive signals from the instrument over time relating to a load impedance between the active electrode and the return electrode of the instrument, determine based on the signals that the active electrode and the return electrode are currently shorted together, and prior to the short, the instrument was grasping tissue between the active electrode and the return electrode, and based on the determination, reduce a current limit of treatment energy being provided to the instrument to below the corresponding current limit.
Medical devices and related methods
A medical device includes a shaft including a central lumen configured to direct a flow of fluid through the shaft, and an electrode positioned at a distal portion of the shaft. The electrode includes an electrode lumen in fluid communication with the central lumen, and the electrode lumen is configured to receive the flow of fluid from the central lumen. The electrode also includes one or more channels angled relative to the electrode lumen, and the one or more channels are in fluid communication with the electrode lumen to receive the flow of fluid from the electrode lumen. The one or more channels are configured to divert the flow of fluid from the electrode lumen toward one or more outlets laterally offset from the electrode lumen.

A precision forward-looking image-guided diagnostic and therapeutic surgical probe and needle insert for microsurgery in support of imagery, neurology, neurosurgical procedures, and ophthalmic surgical applications comprising an introducer needle (stylet), a fiber carrier, a therapeutic conduit, and a spirographic method for scanning a target and associated algorithms to create and render a reconstructed image for display to a physician in real-time or near real-time. The probe implements Optical Coherence Tomography (OCT) to provide high-resolution extended imagery of an intended therapeutic or target tissue. A separate therapeutic conduit provides surgical access for therapeutic devices such as a cutting or ablation laser, an RF electrode for locally heating tissue, a lumen for local injection of neurolytics/paralytics, placement of electrodes for neuromodulation, and deployment of a micro-endoscopic imaging tool. A third working channel supports the delivery of neurolytic and other fluids.

ELECTROSURGICAL APPARATUS FOR TREATING BIOLOGICAL TISSUE WITH MICROWAVE ENERGY
Various embodiments provide an electrosurgical apparatus for treating biological tissue with microwave energy. The apparatus comprises: a microwave energy signal generator for generating a microwave energy waveform; an electrosurgical instrument arranged to deliver the microwave energy waveform from a distal end thereof for tissue treatment; and a controller in communication with the microwave energy signal generator. The microwave energy signal generator is configured to deliver the microwave energy waveform as one microwave energy signal pulse. The controller is configured to control the profile of the one microwave energy signal pulse to cause ablation or coagulation of the biological tissue and to substantially prevent the one pulse from causing heat to build-up in the electrosurgical instrument.
