Control methods and devices for energy delivery

Control systems and methods for delivery of energy that may include control algorithms that prevent energy delivery if a fault is detected and may provide energy delivery to produce a substantially constant temperature at a delivery site. In some embodiments, the control systems and methods may be used to control the delivery of energy, such as radiofrequency energy, to body tissue, such as lung tissue.

Estimating a temperature during ablation

A method for use with an intra-body probe, a distal end of which includes an ablation electrode and a temperature sensor, is described. While (i) the ablation electrode is driving an ablating current into tissue of a subject, and (ii) fluid is passed from the distal end of the intra-body probe at a fluid-flow rate, a processor receives a temperature sensed by the temperature sensor. The processor estimates a temperature of the tissue, based at least on the sensed temperature and at least one parameter selected from the group consisting of: the fluid-flow rate, and a parameter of the ablating current. The processor generates an output in response to the estimated temperature. Other embodiments are also described.

A method and apparatus for providing hand-mounted surgical tools is provided. The apparatus includes a housing configured to be mounted to a body of a user. The apparatus also includes an optical source to generate a first optical signal in an absorption spectrum of a biocompatible fluorescing dye (BFD). The apparatus also includes an optical detector to detect a second optical signal in an emission spectrum of the BFD. The apparatus also includes a processor to receive a signal from the optical detector that indicates that the second optical signal was detected by the optical detector. The processor is further configured to cause the apparatus to transmit a signal to a non-visual feedback device to cause the non-visual feedback device to output non-visual feedback to the user that the second optical signal was detected by the optical detector.

REAL-TIME SURGICAL TOOL PRESENCE/ABSENCE DETECTION IN SURGICAL VIDEOS

Embodiments described herein provide various techniques and systems for building machine-learning surgical tool presence/absence detection models for processing surgical videos and predicting whether a surgical tool is present or absent in each video frame of a surgical video. In one aspect, a process for ensuring patient safety during a laparoscopic or robotic surgery involving an energy tool is disclosed. The process can begin receiving a real-time control signal indicating an operating state of an energy tool during the surgery. Next, the process receives real-time endoscope video images of the surgery. The process simultaneously applies a machine-learning surgical tool presence/absence detection model to the real-time endoscope video images to generate real-time decisions on a location of the energy tool in the real-time endoscope video images. The process then checks the real-time control signal against the real-time decisions to identify an unsafe event and takes a proper action when an unsafe event is identified.

The impedance, correlated to the temperature, may be set at a desired level, such as a percentage of initial impedance. The set impedance may be a function of the initial impedance, the size and spacing of the electrodes, the size of a targeted passageway, and so on. The set impedance may then be entered into a PID algorithm or other control loop algorithm in order to extract a power to be applied to a treatment device.

The instructions when executed, cause the generator to provide an indicated treatment energy to the instrument, where the indicated treatment energy is set by a user and having a corresponding current limit, receive signals from the instrument over time relating to a load impedance between the active electrode and the return electrode of the instrument, determine based on the signals that the active electrode and the return electrode are currently shorted together, and prior to the short, the instrument was grasping tissue between the active electrode and the return electrode, and based on the determination, reduce a current limit of treatment energy being provided to the instrument to below the corresponding current limit.

Provided herein are systems for treating tissue of a patient. The system comprises an energy delivery console and at least one energy delivery device. The energy delivery console can provide a first dose of energy and a second dose of energy. An energy delivery device comprises a first delivery element configured to deliver the first dose of energy to target tissue, and a second delivery element configured to deliver the second dose of energy to the target tissue. The first dose of energy can comprise a delivery of energy that reversibly alters the target tissue, and the second dose of energy can comprise a delivery of energy that irreversibly alters the target tissue. The first dose of energy can be delivered to enhance a therapy provided by the second dose of energy.

In a case where no error is detected, the method includes implementing a tissue treating algorithm to treat tissue grasped between the first and second jaw members. The tissue treating algorithm includes conducting electrosurgical energy between the first and second jaw members and through tissue grasped therebetween. In a case where an error is detected, the method includes determining, based on additional feedback data, a cause of the error, and outputting an alarm indicating the error and the cause of the error.
