Disclosed herein are systems and methods for locating and identifying nerves innervating the wall of arteries such as the renal artery. The present invention identifies areas on vessel walls that are innervated with nerves; provides indication on whether energy is delivered accurately to a targeted nerve; and provides immediate post-procedural assessment of the effect of energy delivered to the nerve. The methods include evaluating a change in physiological parameters after energy is delivered to an arterial wall; and determining the type of nerve that the energy was directed to (sympathetic or parasympathetic or none) based on the evaluated results. The system includes at least a device for delivering energy to the wall of blood vessel; sensors for detecting physiological signals from a subject; and indicators to display results obtained using said method. Also provided are catheters for performing the mapping and ablating functions.

Systems and methods for use in measuring sympathetic nervous system activity or blood vessel autoregulation corrected for sympathetic activity. The choroid in the human eye is imaged and, using the resulting image, the vascular perfusion density (VPD) in the choroid is measured. VPD provides a measurement that is directly related to sympathetic nervous system activity. The effect of stimuli on sympathetic nervous system activity can be measured by comparing pre-stimuli VPD measurements with post-stimuli measurements. Quantifying VPD can be performed by determining pixel density within specific areas of the choroid image. Altered sympathetic nervous system activity can be detected in a subject by comparing that subject's VPD measurements to baseline VPD measurements from healthy individuals. Blood vessel autoregulation can be measured by imaging changes in other blood vessels in the eye and correcting with choroid VPD measurements of sympathetic activity.

A system configured in accordance with embodiments of the present technology can include, for example, a detector attached to or implanted in a patient and a receiver communicatively coupled to the detector. The detector can measure cardiac data and the receiver and/or a device communicatively coupled thereto can analyze the cardiac data to provide one or more SNS tone indicators. The SNS tone indicators can be used to determine whether a patient will be responsive to a neuromodulation therapy and/or whether a neuromodulation therapy was effective.

For example, the method may include: receiving a pulse signal obtained during an orthostatic test performed from a sensor placed on the individual; identifying a position and an amplitude of the dicrotic notch for pulses in a portion of the pulse signal to obtain a set of dicrotic notch positions and amplitudes; generating an initial orthostatic response curve from the set of dicrotic notch positions and amplitudes; evaluating the initial orthostatic response curve to obtain an assessment of the initial orthostatic response; and displaying, storing and/or transmitting at least one of the initial orthostatic response curve and a visual representation of the assessment of the initial orthostatic response curve.

An intravascular catheter for peri-vascular nerve activity sensing or measurement includes multiple needles advanced through supported guide tubes (needle guiding elements) which expand with open ends around a central axis to contact the interior surface of the wall of the renal artery or other vessel of a human body allowing the needles to be advanced though the vessel wall into the perivascular space. The system also may include means to limit and/or adjust the depth of penetration of the needles. The catheter also includes structures which provide radial and lateral support to the guide tubes so that the guide tubes open uniformly and maintain their position against the interior surface of the vessel wall as the sharpened needles are advanced to penetrate into the vessel wall. The addition of an injection lumen at the proximal end of the catheter and openings in the needles adds the functionality of ablative fluid injection into the perivascular space for an integrated nerve sending and ablation capability.
