EXPANDABLE SURGICAL ACCESS PORT

An expandable access port includes a port body transitionable between expanded and collapsed configurations. The expandable access port also includes a plug with a first portion secured to the port body and a second portion flexibly coupled to the first portion by an arm. The first portion has an orifice and the second portion has a post insertable into the orifice. A tube is attached to an inflation port on the first portion of the plug. The tube is attachable to a source of inflation fluid and in fluid communication with a chamber of the port body. A lumen extends through the port body and is configured to slidably receive the post or a surgical instrument therethrough. The lumen forms a fluid-tight boundary when the port body is in the expanded configuration.

A dilator system for arterial and venous cannulas, where there is a smooth transition between the dilator and the outer wall of the cannula. One aspect of the subject technology provides a blunt-tip cannula that is used as a dilator for insertion of an arterial or venous cannula. The balloon might either be a single diameter or may have a larger diameter at the distal portion such that the diameter of the distal portion of the balloon dilator matches the diameter of the cannula at the tip. Another aspect of the subject technology is a balloon dilator for cannulae that may be positioned over a guidewire, for guidewire-directed placement of a cannula within a vessel, duct, lumen or heart structure of the body. Another aspect of the subject technology is an inflation system for the balloon dilator, such that the balloon dilator may be deflated and quickly removed from the cannula after the cannula has been positioned and secured within the vessel, duct, lumen or heart structure. Another aspect of the subject technology includes a stiffening element in the balloon dilator along its shaft to increase the rigidity of the combined balloon dilator and cannula to provide improved ease of insertion. Another aspect of the subject technology is an improved curvature of the shape of the right angle dilator that achieves more uniform flow within the dilator and ease of insertion and more even flow within the cannula, and ease of insertion of and retraction of the balloon dilator. Another aspect of the subject technology is a gel-filled balloon dilator that improves ease of insertion of the cannula within the vessel, duct, lumen or heart structure, and can be removed without deflation.

Though not required, the puncture member can have one or more of novel features, including a tapered coil portion, a transition cutter, a gripper coil portion, an atraumatic guard, and a shaped cutting wire. Methods for transseptal punctures and crossing include adding rotation, oscillation and/or ultrasound to a transseptal puncture device.

In one embodiment, the present disclosure relates to a probe for use in minimally invasive surgery. The probe includes a shaft and a tip extending from the shaft. The tip includes a bulbous shape with a smooth and rounded outer surface. In some embodiments, the shaft is separated from the tip by a neck such that both the shaft and the tip expand outward from a concave surface of the neck. In some embodiments, one or both of a neuromonitoring element and an ultrasound sensor are disposed on the tip. In some embodiments, a probe may include a surface feature that resists expulsion of the probe from a human body when the probe is disposed in the human body. In further embodiments, a probe may include insulated material over a first surface region and conductive material over a second surface region.
