Certain aspects of the present disclosure provide methods and apparatus for managing an esophagus of a subject during a medical procedure, such as cardiac tissue ablation or bronchial tissue ablation. Managing the esophagus may include displacing the esophagus, imaging the esophagus, and/or measuring temperature at one or more locations inside the esophagus. One example esophageal management system generally includes a tube configured for insertion through a mouth and into the esophagus of the subject. The tube generally includes a first port located at a proximal end of the tube and in fluid communication with a distal portion of the tube via a first path, a second port located at the proximal end of the tube, and a third port located between the proximal end of the tube and a distal end of the tube and in fluid communication with the second port via a second path.

A balloon is provided for selectively moving an esophagus away from an ablation site. The balloon is received through an oral cavity and into the esophagus of a patient. A deflecting member is provided in the tube, the balloon, or both, to selectively distort to bend the balloon and/or the tube to move the esophagus away from the ablation site. The deflecting member may comprise at least one of a strip made of a shape memory material that is responsive to the receipt of a stimulus to deflect to a predetermined shape, a strip that is made of or contains a ferrous material and that deflects in response to the presence of a magnetic field, and a selectively tensionable cable, wire, or string. The deflecting member may be supplemented by a stiffening strip that is located in the balloon and that causes the balloon to expand circumferentially and asymmetrically when inflated.

SURGICAL GUIDE AND METHOD OF USE

A surgical guide and methods of using the surgical guide are disclosed. The surgical guide includes a body having a first side and a second side. An adhesive material is disposed over at least a portion of the first side. A radiopaque guide element coupled to a second side of the body and an incision guard defines an opening extending from the first side to the second side of the body. The incision guard is sized and configured to receive a surgical instrument therethrough such that the incision guard prevents contact between the surgical instrument and a portion of a surgical site adjacent to the opening.

An improved gastric tube for displacing a section of an esophagus during cardiac ablation procedures is disclosed. The improved gastric tube is an elongated flexible tube designed to be inserted in the esophagus of a patient and extended past the portion of the esophagus which overlies the heart. The improved gastric tube includes a first lumen extending the length of the tube which receives a control wire, plastic stylet, or other apparatus which would function for displacement of the portion of the esophagus overlying the heart. A second lumen is included which extends to the operative section of the gastric tube, where the esophagus overlies the heart, so that contrast liquid or cooling liquid can be injected into the esophagus at that location. A temperature sensor can also be included to measure the temperature of the esophageal wall, as well as electrodes to connect to a three-dimensional mapping system.

A positioning device configured for introduction within a body lumen. Some embodiments include a catheter shaft with a longitudinal axis; an expandable element coupled along a length of the shaft, the expandable element being substantially thin and flexible in an unexpanded state and exerting a gentle outward force in an expanded state; and a deflection mechanism located within the shaft, wherein the deflection mechanism is flexible when in a non-deflected state and wherein, in the deflected state, the deflection curves in a predetermined lateral direction such that the positioning device laterally deflects the body lumen. In some embodiments, the deflection is a lateral curve that moves the body lumen away from a surgical site, such as a cardiac ablation site.
