To solve the problem of making available a speech valve for persons who have undergone a laryngectomy or tracheotomy, which speech valve in particular has a long useful life, a speech valve (10) is proposed having a housing (12) with a proximal opening (14) and at least one filter (70) at least partially received in the housing, and with a piston-like valve element (30), wherein the filter at least partially surrounds the valve element, wherein the valve element is movable on a valve seat (32), arranged on the proximal opening, in order to produce a closure position and can be returned with a spring action to an open position of the speech valve, and wherein the valve element has a lower end (38) which faces towards the proximal opening and which is assigned to the valve seat in the open position and, with the valve seat, forms a gap (40) through which an air stream (17a) can be guided.

A tracheostoma device holder for holding a tracheostoma device superimposed of a tracheostoma of a person is provided. The tracheostoma device holder comprises a rigid plate for attachment over a tracheostoma via a proximal side thereof, the plate being provided with a through hole, and a tubular tracheostoma device fitting, arranged circumferentially of the through hole, the tubular tracheostoma device fitting extending distally from a distal side of the plate. The plate is manufactured in a thermoplastic material, with a melting/softening temperature allowing for molding according to the local anatomic shape of the person through direct contact between the plate and the skin of the person. A method for adaptation of such tracheostoma holding device is also provided.

A medical appliance for the treatment of one or more sleep disorders such as obstructive sleep apnea in a patient, the appliance comprising: a biasing member for inserting behind and exerting a force upon the patient's soft palate or tongue, wherein the biasing member is inserted in a reduced or minimized form and then expanded or firms once in place to exert the force. The appliance may be nasally inserted or be placed through the mouth. In a particular configuration, both the soft palate and tongue are biased to prevent obstruction of the flow of air in the nasopharyngeal airway.

A speech valve for placing and fitting on a tracheostomy cannula, having a throughflow body enclosing a throughflow channel which on one side at one end has a connection for joining to a proximal end of the tracheostomy cannula, a one-way valve located at an end of the throughflow body opposite the connection, and, at a distance along the throughflow channel between the connection and the one-way valve, a valve opening is located which is closable by an overpressure valve. The overpressure valve arrangement has at least one elastically deformable valve flap which, in a first state covers the valve opening provided between the connection structure and the one-way valve on the throughflow body and, at a predefinable overpressure occurring inside the throughflow channel converts, by an overpressure-driven elastic deformation of the valve flap, to a second state, in which it at least partially exposes the valve opening.

Said tubular being at the proximal end of the tracheostoma device, comprising at least one inlet upon exhalation through the tracheostoma device, and being adapted for cooperation with the tracheostoma device holder. The container being at the distal end of the tracheostoma device, comprising at least one outlet upon exhalation through the tracheostoma device, and being adapted for housing a heat and moisture exchanging foam. The tubular body and the container may have a hardness of less than and including 100 Shore A.
